with all the tools necessary to evaluate or develop your<br />

own health home model, without recreating the wheel.<br />

exist to adequately serve the marginalized LGBTQ population’s<br />

needs. Explore a recovery support model built<br />

upon evidence-based literature and three years of field<br />

research to better meet the needs of youth who selfidentify<br />

as LGBTQ. Learn to reach and better provide recovery<br />

support and services to youth and young adults<br />

experiencing homelessness who self-identify as LGBTQ.<br />

evidence-based practices to serve older adults?<br />

Do you know how to navigate Medicare and otherwise<br />

get paid for older adults services? With an aging baby<br />

boom generation, behavioral health organizations must<br />

prepare to meet this population’s needs. Get to know<br />

the evidence-based practices available and learn from a<br />

program already sustaining a continuum of care for older<br />

adults with behavioral health problems.<br />
