In general (continued) Pregnancy • Standard: Unless a pregnant woman with –ve cytology has gone beyond 3 years without having cervical screening then the test should be postponed. • But next standard: If a woman has been called for routine screening and she is pregnant then the test should be deferred. • Confusing. So remove 1 st standard. • Add another standard: ‘If a woman requires colposcopy or cytology after treatment (or follow up of untreated CIN1) and is pregnant then that assessment can be delayed until after delivery. However if a 1 st follow up cytology/ colposcopy is required following treatment for CIN2/3 with involved or uncertain margin status or treatment for cGIN then that visit should not be delayed (unless there is an obstetric contraindication).’ • ‘The colposcopist may wish to perform only colposcopy at a follow up appointment in pregnancy.’ • ‘If repeat cytology is due and the woman has missed or defaulted her appointment prior to pregnancy consideration should be given to that woman having her cytology or a colposcopy during pregnancy.’
In general (continued) Clinic staffing • There must be 2 nurses for each clinic...add ‘These roles are interchangeable so that 2 nd nurse could be the clinic assistant and the primary nurse could work outside the colposcopy room but be available for all supporting activities as and when required.’ • ‘All colposcopists require support staff in the colposcopy room for the duration of the session.’ • Nurse colposcopists require same level of support.
