Withdrawal from cervical screening<br />
• Women with severe cervical stenosis (usually result of previous<br />
surgery) may not be able to have cytology which is representative of<br />
whole TZ.<br />
• Options are:<br />
– hysterectomy<br />
– cervical dilatation<br />
– withdrawal from further cervical screening.<br />
• Cervical dilatation should be considered in all cases. For women<br />
with history of high-grade CIN/ cGIN or unexplained high-grade<br />
cytology then cervical dilatation/ hysterectomy recommended.<br />
Involvement of colposcopy MDT can be useful. Otherwise<br />
withdrawal suggested.<br />
• Local call/ recall service must be informed of management decision<br />
in all cases.<br />
• Withdrawal can be recommended by lead colposcopist by letter to<br />
local call/ recall service on behalf of woman as long as she agrees<br />
to withdrawal <strong>and</strong> both she <strong>and</strong> her GP receive written confirmation<br />
of withdrawal from cervical screening.<br />
• If woman declines withdrawal advised by local colposcopy service<br />
then cervical screening should continue.
