This disease does not show AD or AR inheritance. Sudden death may be the first clinical presentation in these subjects and may occur also in adult or mature age. The mechanism of SCD is the development of atrial fibrillation with rapid atrioventricular conduction via the bypass tract, resulting in rapid ventricular response and subsequent ventricular fibrillation.
The resting ECG may demonstrate an initial slurred upstroke of the QRS complex (delta wave), short PR interval, and wide QRS complex. The vast majority of WPW patients are diagnosed by the resting 12-lead ECG pattern. However, the preexcitation pattern may be present only occasionally in the resting 12lead ECG, due to changes in the accessory pathway refractoriness.
