2 ANALYSIS<br />
2.1 Evidence<br />
From 9 to 12 June 2007, two investigators from <strong>the</strong> Australian Transport Safety<br />
Bureau (ATSB) collected relevant evidence.<br />
The investigators attended <strong>the</strong> grounded <strong>Pasha</strong> <strong>Bulker</strong> and <strong>the</strong> <strong>of</strong>fices <strong>of</strong> <strong>the</strong> legal<br />
representative <strong>of</strong> <strong>the</strong> ship’s owners. The master, all three mates, helmsman and<br />
chief engineer were interviewed and provided <strong>the</strong>ir accounts <strong>of</strong> <strong>the</strong> incident. The<br />
data card from <strong>the</strong> ship’s voyage data recorder (VDR) was obtained. Copies <strong>of</strong> log<br />
books, bell book, <strong>the</strong> engine movement logger, wea<strong>the</strong>r reports, ship’s certificates,<br />
plans, stability information and o<strong>the</strong>r relevant documents were also collected.<br />
At Newcastle Port Corporation (NPC), <strong>the</strong> harbour master was interviewed. At<br />
Newcastle Vessel Traffic Information Centre (VTIC), <strong>the</strong> three involved VTIC<br />
<strong>of</strong>ficers were interviewed. Copies <strong>of</strong> radar and wea<strong>the</strong>r monitoring records, events<br />
log, procedures and documents from <strong>the</strong> centre were also taken.<br />
On 18 June, whilst Sea Confidence was ber<strong>the</strong>d in Newcastle, <strong>the</strong> investigators<br />
interviewed <strong>the</strong> ship’s master.<br />
On 8 July, <strong>the</strong> master <strong>of</strong> Betis was also interviewed by <strong>the</strong> investigators after <strong>the</strong><br />
ship had ber<strong>the</strong>d in Newcastle.<br />
On 23-24 August, <strong>the</strong> investigators obtained fur<strong>the</strong>r relevant information through<br />
meetings and interviews with Port Waratah Coal Services (PWCS) staff, ships<br />
agents in Newcastle, NPC and its pilots.<br />
During <strong>the</strong> course <strong>of</strong> <strong>the</strong> <strong>investigation</strong>, information was also obtained from several<br />
o<strong>the</strong>r sources including Svitzer Salvage.<br />
Salvage operations to refloat <strong>Pasha</strong> <strong>Bulker</strong> were also attended by <strong>the</strong> ATSB.<br />
During <strong>the</strong> <strong>investigation</strong>, <strong>the</strong> ATSB conducted a survey <strong>of</strong> <strong>the</strong> masters <strong>of</strong> ships in<br />
<strong>the</strong> Newcastle anchorage at <strong>the</strong> time <strong>of</strong> <strong>the</strong> incident. The results <strong>of</strong> <strong>the</strong> survey are<br />
included in <strong>the</strong> report (Appendix E).<br />
On 19 December, at a meeting with ATSB investigators, fur<strong>the</strong>r information was<br />
provided by NPC and PWCS staff.<br />
2.2 Decisions made on board <strong>Pasha</strong> <strong>Bulker</strong><br />
The <strong>grounding</strong> <strong>of</strong> <strong>Pasha</strong> <strong>Bulker</strong> occurred in extreme wea<strong>the</strong>r conditions. However,<br />
<strong>the</strong>re were a number <strong>of</strong> decisions that were made on board <strong>the</strong> ship from 7 June<br />
onwards which, in <strong>the</strong> circumstances and <strong>the</strong> wea<strong>the</strong>r, contributed significantly to<br />
its <strong>grounding</strong>.<br />
2.2.1 At anchor<br />
When <strong>the</strong> ship arrived <strong>of</strong>f Newcastle on 23 May, <strong>Pasha</strong> <strong>Bulker</strong>’s master anchored<br />
<strong>the</strong> ship in a position consistent with <strong>the</strong> standard anchoring advice from VTIC. At<br />
<strong>the</strong> time, <strong>the</strong> wea<strong>the</strong>r was good, <strong>the</strong> ship’s distance from <strong>the</strong> shore was more than<br />
