Environmental Stewardship<br />
at Kaiser Permanente<br />

Kathy Gerwig is the Director of Environmental Stewardship <strong>and</strong> National<br />
Environmental Health <strong>and</strong> Safety for Kaiser Permanente, a non-profit, group practice<br />
prepayment program with headquarters in Oakl<strong>and</strong>, California. It includes the Kaiser<br />
Foundation Health Plan, Inc., Kaiser Foundation Hospitals, <strong>and</strong> the Permanente<br />
Medical Groups.<br />
Environmental activism at Kaiser<br />
Permanente began four decades ago<br />
when the organization invited Rachel<br />
Carson to deliver the keynote address to a<br />
large symposium of physicians <strong>and</strong><br />
scientists. Her book, Silent Spring, which<br />
has been widely credited for launching<br />
today’s environmental movement, had just<br />
been released. Her keynote address at the<br />
Kaiser gathering was her last public<br />
appearance before her death from breast<br />
cancer.<br />
The Paradox<br />
The environmental paradox for health care<br />
is that in the course of providing health care<br />
to individuals, health care institutions use<br />
chemicals <strong>and</strong> materials that are hazardous<br />
to human health.<br />
Two examples are:<br />
• Medical waste incinerators that are<br />
among the highest contributors of<br />
dioxin in the air we breathe. Burning<br />
polyvinyl chloride creates dioxin, <strong>and</strong><br />
polyvinyl chloride is ubiquitous in<br />
healthcare.<br />
• Mercury thermometers, one of which is<br />
enough to contaminate a 20-acre lake.<br />
Heavy metals are found in<br />
manometers, fixatives, <strong>and</strong> a variety of<br />
other healthcare products. They are<br />
persistent, <strong>and</strong> they bioaccumulate in<br />
the environment.<br />
The only responsible course of action is to<br />
eliminate or reduce these hazards.<br />

Vision for Environmental<br />
Stewardship<br />
Kaiser Permanente’s (KP’s) organizational<br />
mission is to improve the health of the<br />
communities it serves. That mission extends<br />
to the health of the environment because<br />
healthy people require clean air, water, <strong>and</strong><br />
soil.<br />
KP’s vision for environmental stewardship<br />
is to aspire to provide health care services<br />
in a manner that protects <strong>and</strong> enhances the<br />
environment <strong>and</strong> the health of communities<br />
now <strong>and</strong> for future generations.<br />
