Away from Home Care ®<br />
Your HMO Coverage Goes With You<br />
You have access to routine and urgent care when you’re<br />
away from home for 90 consecutive days or more.<br />
Whether you’re out of town on extended business,<br />
travel or attending school out of the area, you’ll have<br />
access to your HMO benefits.<br />
Coverage Away from Home<br />
You’ll have coverage when you see a provider of an<br />
affiliated Blue Cross and Blue Shield HMO (Host HMO)<br />
outside of the CareFirst BlueChoice, Inc. service area<br />
(Maryland, Washington, D.C., Northern Virginia). When<br />
you receive care, you’ll be considered a member of the<br />
Host HMO and will be responsible for the copay benefits<br />
under that plan. Your copay benefits may be different<br />
than when you’re in the service area.<br />
No Paperwork or Up-front Costs<br />
You won’t have to complete claim forms and are only<br />
responsible for out-of-pocket expenses such as copays,<br />
deductibles, coinsurance, and the cost of non-covered<br />
services. CareFirst BlueChoice, Inc. will communicate<br />
this information to you when your Away from Home<br />
Care application has been accepted<br />
Enrolling in Away from Home Care<br />
To ensure you and your family have ongoing access to<br />
the care you need:<br />
Contact Member Services at the number on your<br />
ID card and ask for the Away from Home Care<br />
Coordinator.<br />
We’ll let you know if there’s a Host HMO in the area<br />
where you’ll be staying and we’ll work with you to<br />
complete an application.<br />
The completed application will be sent to you for<br />
your signature and date. Once it’s returned, we’ll<br />
forward it to the Host HMO at your destination.<br />
The Host HMO will provide an ID card, a primary<br />
care provider (you may be asked to choose your<br />
own primary care provider), and information on how<br />
to access your benefits while using your Away from<br />
Home Care.<br />
When you need routine care, just call your Host<br />
HMO primary care provider for an appointment.<br />
