CareFirst Prescription Drug Card Plan<br />

For PPO & BlueChoice HMO Plans<br />

Your pharmacy benefit program is now administered by<br />

Argus Health Services. This program is based on the<br />

CareFirst preferred drug list, formerly formulary, that<br />

encourages the use of generic drugs and certain brand<br />

name drugs. You pay a different copay depending on<br />

whether you choose a generic drug, a brand-name drug<br />

on the preferred drug list, or a non-preferred brandname<br />

drug. Always remember to talk to your doctor<br />

about using preferred drugs that can save you money.<br />

You and your doctor should check your preferred drug<br />

list before you receive a prescription.<br />

Retail Program<br />

The retail program provides a 34-day or less supply<br />

of medication when purchased at a participating<br />

retail pharmacy. Present your prescription drug<br />

identification card at any participating pharmacy and<br />

pay the appropriate copayment for your medication.<br />

Maintenance medication when purchased at a<br />

participating pharmacy is dispensed up to a 90-day<br />

supply for one copay for PPO PLUS members, two<br />

copays for PPO CORE Plan members and three copays<br />

for HMO Plan members.<br />

Mail Order Service<br />

Prescription Program<br />

Your mail order prescription drug program is administered<br />

by Walgreens Mail Order Services. The Mail Order Service<br />

Prescription Program is a special added feature to your<br />

CareFirst Plan. For those who regularly take one or more<br />

types of maintenance medication, this service provides<br />

a convenient, inexpensive way for you to order these<br />

medications and have them delivered at home.<br />

For PPO PLUS, you can order up to a 90-day supply<br />

of maintenance medication for the $20 copayment.<br />

For PPO CORE, you can order up to a 90-day supply<br />

of maintenance medication for 1 times the required<br />

copayment ($10/25/40). For HMO, you can order a<br />

90-day supply of maintenance medication for 2 times<br />

the copayment ($5/15/35). The copayment cannot be<br />

reimbursed through your Medical <strong>Benefits</strong> Plan.<br />

Medications are delivered to your home postage paid<br />

via UPS or First Class U.S. Mail.<br />
