by any in-network specialist.<br />

Non-participating Specialist care requires Pre-authorization by the <strong>Health</strong> Services Department. A request for<br />

Out-of Network services can be initiated by calling <strong>Sendero</strong>’s <strong>Health</strong> Service’s Department at 1-855-297-9191<br />

or by faxing a request with the appropriate documentation to justify the request to 1-512-275-2862.<br />

Physician-requested Second Opinions and Member-requested Second Opinions<br />

Second opinions requested by either the Member or the physician do not require pre-authorization. For<br />

questions regarding a second opinion request, contact the <strong>Health</strong> Services Department.<br />

Results of Not Obtaining Pre-authorization<br />

Cases that require pre-authorization and in which pre-authorization was not obtained are subject to denial.<br />

Appeal information can be found in “ST5 Complaints & Appeals” or “CH4 Complaints & Appeals” in this<br />

manual.<br />

Appealing Non-Payment for Lack of Referral<br />

Online Referrals and Authorization Processes<br />

Request for authorization of outpatient services can be initiated on line. Once you have accessed<br />

www.<strong>Sendero</strong><strong>Health</strong>.com, click on the link for the Provider Portal … Once in the Provider section of the<br />

website, select the link on the left for creating an Online Referral or for initiating the pre-authorization process<br />

depending on your need. .<br />

Faxing Paper Referrals and Authorization Requests<br />

Providers may fax the <strong>Sendero</strong> Authorization and Referral Form or the Texas Authorization and Referral Form<br />

(in Appendix A) to the <strong>Health</strong> Services Department at 512-275-2862.<br />

Obtaining Referral and Authorization Forms<br />

Forms are available online as well as from the <strong>Health</strong> Services Department by clicking on the following link<br />

or by typing the web address into your browser.<br />

6.4 Pre-Authorization<br />

Overview<br />

<strong>Sendero</strong> <strong>Health</strong> <strong>Plans</strong> requires notification only for most planned admissions and procedures so that discharge<br />

planning can be facilitated timely. For certain admissions and / or procedures (such as those that may be<br />

interpreted as “cosmetic” in nature), <strong>Sendero</strong> does require pre-authorization. A list of these situations can be<br />

located in Appendix A of this manual or by accessing the list via this internet link to <strong>Sendero</strong>’s website, <<br />

pending HHSC approval>. The pre-authorization process is used to evaluate the medical necessity of a<br />
