Once contacted, the Call Center will conference call the Member’s primary care provider’s office to help<br />

in scheduling the appointment, while the Member is on the line.<br />

The Call Center will send out an appointment reminder letter to the Member once the appointment is<br />

made.<br />

If unable to reach Member/parent by telephone, the Call Center will send them a postcard to remind<br />

them to call their primary care provider to schedule their child’s Texas <strong>Health</strong> Steps visit that is due.<br />

In addition, the Call Center helps Migrant Farm Worker’s children with acceleration of services prior to<br />

leaving the area, if needed.<br />

The Call Center reaches out to help schedule Texas <strong>Health</strong> Steps appointments for Members that are due for a<br />

visit.<br />

ST 4.18 Pregnancy Notification Requirements<br />

Pregnant STAR/Medicaid Members<br />

<strong>Sendero</strong> <strong>Health</strong> Services Department should be notified as soon as the Member is determined to be pregnant, as<br />

well as advised of any high risk factors. This will allow the RN Care Coordinators to work collaboratively with<br />

the physician and provide proactive case management in order to help in maintaining a healthy full term<br />

pregnancy.<br />

Obtaining Pregnancy Notification Forms<br />

Supplies of Pregnancy notification forms are available to provider’s offices, or see Appendix A of this manual.<br />

Contact Network Management at the phone number at the bottom of this page for information regarding these<br />

forms. These forms may be completed and faxed to the <strong>Health</strong> Services Department at 512-275-2862 to notify<br />

<strong>Sendero</strong> of a Member’s pregnancy.<br />
