of <strong>Sendero</strong> more rapidly and unpredictably than the general population, experiencing placements and<br />

reunification inside and out of the Service Area. During the transition period and beyond, providers must:<br />

o Provide medical records to DFPS<br />

o Schedule medical and behavioral health appointments within 14 days unless requested earlier by<br />

DFPS<br />

o Participate, when requested by DFPS, in planning to establish permanent homes for Members<br />

o Refer suspected cases of abuse or neglect to DFPS<br />

For help with Member and DFPS, providers should call <strong>Sendero</strong>’s <strong>Health</strong> Services Department.<br />

Essential Public <strong>Health</strong> Services<br />

<strong>Sendero</strong> is required through its contractual relationship with HHSC to coordinate with Public <strong>Health</strong> Entities<br />

regarding provision of services for essential public health services. Providers must assist <strong>Sendero</strong> in these<br />

efforts by:<br />

• Complying with public health reporting requirements regarding communicable diseases and/or diseases<br />

which are preventable by immunizations as defined by State Law.<br />

• Assisting in notifying or referring to the local Public <strong>Health</strong> Entity, as defined by state law, any<br />

communicable disease outbreaks involving Members<br />

• Referring to the local Public <strong>Health</strong> Entity for TB contact investigation and evaluation and preventive<br />

treatment of person whom the Member has come into contact<br />

• Referring to the local Public <strong>Health</strong> Entity for STD/HIV contact investigation and evaluation and<br />

preventive treatment of persons whom the Member has come into contact<br />

• Referring for Women, Infant, and Children (WIC) services and information sharing<br />

• Assisting in the coordination and follow up of suspected or confirmed cases of childhood lead exposure<br />

• Reporting of immunizations provided to the statewide ImmTrac Registry including parental consent to<br />

share data<br />

• Cooperating with activities required of public health authorities to conduct the annual population and<br />

community based needs assessment<br />

• Referring lead screening tests to the Texas Department of State <strong>Health</strong> Services. To report lead<br />

poisoning, provider can complete the applicable form for Children or Adults and fax it to 512.776-7699,<br />

or call DHSH at 512-776-7269 or toll free at 1-800-588-1248. The following information must be<br />

reported: child’s name, address, date of birth, sex, race and ethnicity; blood lead level concentration,<br />

test date, name and telephone number of testing laboratory; whether the sample was capillary or venous<br />

blood; and the name and city of the attending physician.<br />

<strong>Sendero</strong>’s RN Care Coordinators can help organize services with the Member, the Member’s primary care<br />

provider and the public health entity. In cases where services are coordinated and subject to applicable laws,<br />

rules and regulations concerning confidentiality of certain health information, the public health entity is<br />

requested to provide a written report to the primary care provider concerning the services provided by the public<br />

health entity.<br />
