Student Services Section 504/ADA Section 504 of the Rehabilitation Act of 1973 and the Americans with Disabilities Act of 1990 are legislation designed to prevent discrimination against individuals with disabilities. A student is eligible for a Section 504 Accommodation Plan when a school-based committee finds that a student has a physical or mental impairment which substantially limits one or more major life activities. Eligible students are entitled to accommodations in the regular-education setting or special education with related aids and services. Each school has a Section 504 Designee who should be contacted with any questions or concerns. The District Section 504 Facilitator may also be contacted in the Student Services Department at 407-870-4897 for more information, assistance with school issues, or for a copy of the local Section 504 Grievance Procedure. The Intervention Assistance Team The Intervention Assistance Team is a group of educational professionals at each school who review school-wide data to evaluate the effectiveness of general education core curriculum in order to ensure the provision of high-quality instruction. They also identify students who are struggling with academics and/or behavior. A problem-solving method is used to determine solutions for improving the core curriculum and for identifying appropriate interventions. Students’ progress is monitored at regular intervals to determine the effectiveness of curriculum changes and/or interventions. Parents can contact the Response to Intervention (RtI) Coach at the school if they are interested in assistance for their child. Out-of-Zone Requests The window for out-of-zones is April 1 through June 1. The parent or legal guardian of an elementary, middle, or high school student may request that a student attend a school out of his/her attendance zone. An out-of-zone transfer form may be obtained from the student’s home-zoned school, the school that the student is currently attending, or the district office, located at 817 Bill Beck Boulevard in Kissimmee. Completed forms may be hand carried or mailed to the district office. Schools do not process, deliver, or forward the completed form. All approved out-of-zone requests require the parent to be responsible for transportation. For information regarding out-of-zones, please contact Student Services at 407-870-4897. Truancy Prevention and Intervention Each school has an attendance intervention team that goes to work when a student begins to demonstrate truant behavior (five unexcused absences). They also work closely with the Office of the State Attorney and with the Juvenile Court to assist parents and students in improving attendance, which is vital to academic success. For information regarding attendance standards and procedures in dealing with truant behavior, contact the Department of Student Services at 407-870-4897. Parent Resource Health Information Parent resources are located on the Florida Department of Education website at www.fldoe.org/family/fspsa.asp. Required immunizations and the recommended immunization schedule are located on the Florida Department of Health website at www.doh.state.fl.us/disease_ctrl/immune/index.html. Student health resource information is available via the Florida Healthy Kids Corporation website at www.healthykids.org. Detailed information about meningococcal disease and vaccination is located on the Centers for Disease Control and Prevention (CDC) website at www.cdc.gov/vaccines/pubs/vis/downloads/vis-mening.pdf. Mental Health Services Parents may contact School Social Workers in the Student Services Department at 407-870-4897 or the Systems of Care Specialist at 407-343-8677 for more information on community agencies. Health Services Health Services provides coordinated school-based nursing services for students to promote school attendance and enhance academic achievement. The nurses initiate plans of care and coordinate any necessary medical accommodations to meet their needs. Health Services works with the Department of Health to review immunizations and medical records, and to provide appropriate health screenings. 12
