that would be true. I told her that this included the signature as well. She said that her staff only completes the top<br />

part of the form not the signature area. She asked for a few of the initals on the forms and said that sp,me of those<br />

people no longer worked there. I asked her if her organization suspected any wrong doing what did they do? She<br />

said that the office would complete a incident report and keep it on file. I asked if she also notified the, state or<br />

counties if there was an issue so the forms would be looked at, she hesitated but said yes. I didn't get the feeling<br />

from her that this was true. More to cover the organizations back if anything happened.<br />

Please keep in mind that this is an account from nine months ago so there could be things that are not as<br />

accurate as they would be in the emails I drafted during that time about this situation.<br />
