DeLacy, M.D., Gastroenterology Consultants of South Jersey, P.C., Lumberton, NJ, which is expected to last more than two (2) weeks; and WHEREAS, the following Board of Education comments and/or discussion was had, if any, on the matter of this Resolution: NOW, THEREFORE, BE IT RESOLVED by the Township of Florence Board of Education, County of Burlington, State of New Jersey, that the above homebound instruction is hereby approved at the determined rate of forty three dollars and no cents ($43.00) per hour; and BE IT FURTHER RESOLVED that the homebound instruction extension period of this subject approval shall begin on March 21, 2011 through April 15, 2011, and that any and all extensions shall be approved subject to N.J.A.C. 6:28-4.5 (a) 7. ********** CERTIFICATION OF THE BOARD SECRETARY ********** IT IS HEREBY CERTIFIED that the foregoing Resolution was duly adopted by the Florence Township Board of Education, by the vote below indicated, at the Regular Meeting/School Budget Public Hearing held on Monday, March 28, 2011 in the Florence Township Memorial High School, 1050 Cedar Lane, Florence, New Jersey.
