Exam<strong>in</strong>ation of national surveys revealed disparities <strong>in</strong> all<br />

cardiovascular disease risk factors, hospitalizations for major<br />

cardiovascular disease, overall mortality, and quality of life.<br />

Elim<strong>in</strong>at<strong>in</strong>g these disparities is a major public health challenge <strong>in</strong> the<br />

United States. Their causes and underly<strong>in</strong>g mechanisms, however,<br />

rema<strong>in</strong> <strong>in</strong>completely understood. The healthcare delivery system<br />

itself, access to care, quality of care received, communication barriers,<br />

<strong>in</strong>dividual behaviours, culture and lifestyles, and discrim<strong>in</strong>ation and<br />

bias all play a part. The pursuit of systems and policy changes to<br />

address these determ<strong>in</strong>ants rema<strong>in</strong>s crucial. The authors present a<br />

strategic framework for elim<strong>in</strong>at<strong>in</strong>g health disparities that takes these<br />

determ<strong>in</strong>ants <strong>in</strong>to account.<br />

Many chronic diseases may have their seeds early <strong>in</strong> life.<br />

Observations from the Dutch fam<strong>in</strong>e have taught us that the<br />

<strong>in</strong>trauter<strong>in</strong>e environment is an important determ<strong>in</strong>ant of adult health.<br />

Birth weight has been related to cardiovascular disease (CVD),<br />

hypertension, diabetes, and cancer. Critical phases for adult obesity<br />

development <strong>in</strong>clude time periods between conception and<br />

adolescence. A life course approach to chronic disease prevention<br />

<strong>in</strong>cludes the study of maternal diet dur<strong>in</strong>g pregnancy, <strong>in</strong>trauter<strong>in</strong>e<br />

exposures, postnatal growth, and the adolescent period.<br />

In this article, scenario plann<strong>in</strong>g is applied to public health,<br />

specifically to illustrate the four steps <strong>in</strong> scenario plann<strong>in</strong>g for public<br />

health us<strong>in</strong>g a health department's desire to address chronic disease<br />

prevention and control. <strong>An</strong> unhealthy diet and physical <strong>in</strong>activity are<br />

considered to be key risk factors. The scenarios are presented <strong>in</strong> table<br />

format and are for illustration purposes only. Many other plausible<br />

scenarios could be constructed. Scenario plann<strong>in</strong>g allows stakeholders<br />

to def<strong>in</strong>e a desired, shared vision of the future, but more<br />

important, they can better prepare public health professionals to be<br />

successful <strong>in</strong> a constantly chang<strong>in</strong>g environment.<br />

This article summarizes the state of the science on physical activity<br />

for the overall prevention and treatment of common chronic<br />

diseases, as well as for treat<strong>in</strong>g cancer, improv<strong>in</strong>g diabetes<br />

management, prevent<strong>in</strong>g or revers<strong>in</strong>g osteoporosis, ameliorat<strong>in</strong>g<br />

cardiovascular disease, and achiev<strong>in</strong>g weight loss.<br />

A shortage of credible <strong>in</strong>formation exists on practical dietary and<br />

physical activity patterns that have potential to reverse the national<br />

obesity epidemic and reduce the risk of major cancers and other<br />

chronic diseases. Secur<strong>in</strong>g such <strong>in</strong>formation is a challeng<strong>in</strong>g task, and<br />

there is considerable diversity of op<strong>in</strong>ion concern<strong>in</strong>g related research<br />

designs and priorities. In this article, the authors put forward some<br />

perspectives on useful methodology and <strong>in</strong>frastructure developments
