The Fall Prevention Center of Excellence (Center), a consortium of<br />

federal, state, and private organizations, was established <strong>in</strong> 2005 to<br />

guide the implementation of a state-wide <strong>in</strong>itiative to prevent falls<br />

among older Californians. The Center is currently engaged <strong>in</strong><br />

develop<strong>in</strong>g and dissem<strong>in</strong>at<strong>in</strong>g fall prevention tools and <strong>in</strong>formational<br />

resources directed at the needs of both consumer and professional<br />

audiences; l<strong>in</strong>k<strong>in</strong>g organizations <strong>in</strong>volved <strong>in</strong> fall prevention while<br />

<strong>in</strong>creas<strong>in</strong>g awareness of fall prevention as an important public health<br />

issue; and help<strong>in</strong>g communities build their capacity to effectively<br />

address falls <strong>in</strong> older adults through the delivery of <strong>in</strong>tegrated fall<br />

prevention services and "best practice" programs.<br />

Considerable evidence now documents that the most effective (and<br />

cost-effective) fall reduction programmes have <strong>in</strong>volved systematic<br />

fall risk assessment and targeted <strong>in</strong>terventions, exercise programmes<br />

and environmental-<strong>in</strong>spection and hazard-reduction programmes.<br />

These f<strong>in</strong>d<strong>in</strong>gs have been substantiated by careful meta-analysis of<br />

large numbers of controlled cl<strong>in</strong>ical trials and by consensus panels of<br />

experts who have developed evidence-based practice guidel<strong>in</strong>es for<br />

fall prevention and management. Medical assessment of fall risks and<br />

provision of appropriate <strong>in</strong>terventions are challeng<strong>in</strong>g because of the<br />

complex nature of falls. Optimal approaches <strong>in</strong>volve <strong>in</strong>terdiscipl<strong>in</strong>ary<br />

collaboration <strong>in</strong> assessment and <strong>in</strong>terventions, particularly exercise,<br />

attention to co-exist<strong>in</strong>g medical conditions and environmental<br />

<strong>in</strong>spection and hazard abatement.<br />

This study’s objective was to provide an assessment of the contours<br />

of the <strong>in</strong>jury field today and to raise questions about our future<br />

direction. The authors conclude that the responsibilities of <strong>in</strong>jury<br />

prevention professionals will <strong>in</strong>crease not only because of the<br />

projected <strong>in</strong>crease <strong>in</strong> the global burden of <strong>in</strong>jury but also because of<br />

these professionals’ expanded conceptualization of what the scope of<br />

<strong>in</strong>jury prevention should be. The lack of clarity professionals project<br />

about the substantive areas of their expertise and the <strong>in</strong>commensurate<br />

fund<strong>in</strong>g for efforts represent challenges to the field's coherence and<br />

ultimate effectiveness.<br />

Falls and falls-related <strong>in</strong>juries are a major problem <strong>in</strong> residential aged<br />

care. While there are many factors contribut<strong>in</strong>g to falls <strong>in</strong> this group,<br />

there are also a number of strategies that can help reduce falls and<br />

associated <strong>in</strong>juries. While programs to prevent falls must be<br />

multifactorial and multidiscipl<strong>in</strong>ary, nurses <strong>in</strong> residential aged care will<br />

play a central role <strong>in</strong> sett<strong>in</strong>g up and coord<strong>in</strong>at<strong>in</strong>g such programs. This<br />

paper reviews a broad range of nurs<strong>in</strong>g, medical and allied health<br />

literature. It then synthesizes the research f<strong>in</strong>d<strong>in</strong>gs to outl<strong>in</strong>e a<br />

comprehensive series of recommendations that nurses can use to<br />

improve cl<strong>in</strong>ical practice <strong>in</strong> this area.<br />
