Although trials of multifactorial falls prevention packages have<br />
reported beneficial results, <strong>in</strong> cl<strong>in</strong>ical practice it is important to<br />

characteristics they are will<strong>in</strong>g to modify, and what changes they are<br />
prepared to make to reduce their risk of fall<strong>in</strong>g. Otherwise, there is<br />
the risk that expensive programs are not properly targeted or fail to<br />
achieve maximum participation rates. Further work on the most<br />
robust and pragmatic methods of synthesiz<strong>in</strong>g disparate studies on<br />

The purpose of this article is to describe <strong>in</strong>jury risk and prevention<br />
strategies across the developmental stages of childhood and<br />
adolescence and associated social contexts. View<strong>in</strong>g <strong>in</strong>jury prevention<br />
from this perspective suggests that developmentally appropriate<br />
<strong>in</strong>terventions conducted over several stages may be more likely to<br />
motivate and susta<strong>in</strong> <strong>in</strong>jury prevention behaviour change across a<br />
lifetime than a s<strong>in</strong>gle <strong>in</strong>tervention or a s<strong>in</strong>gle policy change.<br />
This article summarizes the global burden of musculoskeletal <strong>in</strong>juries<br />
and provides several examples of successful programs that have<br />
improved care of <strong>in</strong>juries <strong>in</strong> health facilities <strong>in</strong> low- and middle<strong>in</strong>come<br />
countries. It discusses WHO efforts to build on the country<br />
experiences and to make progress <strong>in</strong> lower<strong>in</strong>g the burden of<br />
musculoskeletal <strong>in</strong>juries globally.<br />
This article reviews ways <strong>in</strong> which the toll from <strong>in</strong>jury can be lowered<br />
through the spectrum of <strong>in</strong>jury control, <strong>in</strong>clud<strong>in</strong>g surveillance,<br />
prevention, and trauma care. There is room for improvement <strong>in</strong> the<br />
application of scientifically based, proved <strong>in</strong>terventions at all po<strong>in</strong>ts <strong>in</strong><br />
the spectrum <strong>in</strong> all countries. The greatest attention is needed <strong>in</strong> lowand<br />
middle-<strong>in</strong>come countries, however, where most of the world's<br />
people live, where <strong>in</strong>jury rates are higher, and where few <strong>in</strong>jury<br />
control activities have yet been undertaken.<br />
The authors review measures that would strengthen exist<strong>in</strong>g efforts to<br />
prevent and treat <strong>in</strong>juries worldwide. There is a need to strengthen<br />
the capacity of national <strong>in</strong>stitutions to do research on <strong>in</strong>jury control;<br />
to design and implement countermeasures that address <strong>in</strong>jury risk<br />
factors and deficiencies <strong>in</strong> <strong>in</strong>jury treatment; and to assess the<br />
effectiveness of such countermeasures. Although much work rema<strong>in</strong>s<br />
to be done <strong>in</strong> high-<strong>in</strong>come countries, even greater attention is needed<br />
<strong>in</strong> less-developed countries, where <strong>in</strong>jury rates are higher, few <strong>in</strong>jury<br />
control activities have been undertaken, and where most of the<br />
world's population lives. In almost all areas, <strong>in</strong>jury rates are especially<br />
high <strong>in</strong> the most vulnerable sections of the community, <strong>in</strong>clud<strong>in</strong>g<br />
those of low socioeconomic status.<br />
This study’s objective was to ga<strong>in</strong> a better understand<strong>in</strong>g of the<br />
community based model for <strong>in</strong>jury prevention. The study was<br />
performed as a structured review of exist<strong>in</strong>g evaluations of <strong>in</strong>jury<br />
prevention programs that employed multiple strategies to target<br />
different age groups, environments, and situations. The results of this<br />
study suggested that there are complex relationships between the<br />
outcome and the context, structure, and process of community-wide
