Form (2647) and a copy of the worker’s consent.*<br />

■ takes the Functional Abilities Form and a<br />

copy of his/her consent to the health professional<br />

■ asks the health professional to complete the<br />

Functional Abilities Form, and<br />

■ returns a completed copy of the Functional Abilities<br />

Form to the employer, and<br />

■ keeps a copy for personal records.<br />

■ completes the form<br />

■ returns the pink and yellow copies to the worker and<br />

■ sends the white copy to the WSIB for payment.<br />

* Remember, the worker’s consent to release functional<br />

abilities information is their signature on any one of the<br />

■ Worker’s Report of Injury (Form 6)<br />

■ Worker’s Claim/Consent Form (Form 1492)<br />

What is a labour market re-entry<br />

(LMR) assessment<br />

If you are unable to take your worker back or if your<br />

worker’s earnings continue to fall below pre-injury<br />

earnings, the WSIB will conduct a labour market re-entry<br />

assessment. This will determine if your worker needs a<br />

labour market re-entry plan to return to work.<br />

What does an LMR assessment<br />

include<br />

In a labour market re-entry assessment, we identify a<br />

suitable employment or business for the worker – a<br />

category of jobs – that is safe, consistent with your<br />

worker’s functional abilities, and reduces or eliminates<br />

loss of earnings resulting from the injury. The worker’s<br />

employment history, transferable skills and aptitudes<br />

are all considered.<br />

What is a labour market re-entry<br />

(LMR) plan<br />

We want your worker to have the skills, knowledge and<br />

abilities to re-enter the labour market in a job that is safe,<br />

consistent with his or her functional abilities, and reduces<br />

or eliminates any loss of earnings resulting from the<br />

work-related injury.<br />

The LMR assessment identifies a suitable occupation<br />

for the worker. This is often called a suitable employment<br />

or business (SEB). We will then work with your worker,<br />

your worker’s doctor and, whenever possible, with you to<br />

prepare a plan to get the worker job-ready. The plan may<br />

include any of the following<br />

■ job search training<br />

■ skills training<br />

■ academic upgrading<br />

■ English as a second language program<br />

■ formal training.<br />

Who pays for LMR services<br />

The WSIB arranges and pays for external suppliers to<br />

provide labour market re-entry services. The associated<br />

costs appear on the Schedule 2 invoice.<br />

28 Labour market re-entry services apply when an employer is unable to<br />

bring an injured worker back to work.
