Prerequisites for Admission to the<br />

Program<br />

A man must be a Catholic in good standing in the<br />

Church and fully initiated in the faith.<br />

He must be at least 32 years of age.<br />

If married, there must be evidence of a stable and<br />

growing marriage relationship. His wife must be<br />

willing to support her husband actively through<br />

formation and in his ministry.<br />

He will minimally have completed the theological<br />

and ministerial requirements for obtaining a<br />

Certificate of Completion in Lay Ministry from the<br />

<strong>Athenaeum</strong>. (Education is evaluated on an<br />

individual case basis.)<br />

He will have demonstrated his ability to respond to<br />

needs for service himself and to inspire others to do<br />

so.<br />

He is endorsed by his present pastor, parish staff<br />

and parish council.<br />

He has taken Tribunal Procurator Training and the<br />

Child Protection Decree workshop.<br />

He has successfully completed a criminal background<br />

check.<br />

Admission<br />

Application must be made to the director of the<br />

Permanent Deacon <strong>Of</strong>fice, Archdiocese of Cincinnati,<br />

100 E. Eighth Street, Cincinnati, OH 45202,<br />

(513) 421-3131. Applications for the program may<br />

be submitted once a man has completed five (5) of<br />

the courses required for the Certificate in Lay<br />

Ministry. A new formation class begins August,<br />

<strong>2010</strong>. <strong>The</strong> deadline for applications for the next<br />

formation class is October 1, 2012.<br />

Applicants must submit various forms, certificates,<br />

transcripts, etc. Each man also takes a battery of<br />

psychological inventories and participates in an<br />

interview with a counseling professional. A home<br />

visit and extended interview with the applicant (and<br />

wife) is conducted by the deacon office staff. All this<br />

information is then evaluated by a team (both clergy<br />

and lay) and letters indicating whether a person<br />

has been accepted or not are then mailed.<br />

Schedule<br />

<strong>The</strong> diaconate formation program ordinarily meets<br />

at the <strong>Athenaeum</strong> of <strong>Ohio</strong>. However, if at least one<br />

third of those enrolled in the program are from the<br />

northern region of the archdiocese, the program<br />

will relocate to a northern area site for one quarter<br />

of each year.<br />

During the quarters the diaconate formation<br />

program meets at the <strong>Athenaeum</strong>, classes are held<br />

on five weekends during the quarter, beginning<br />

Saturday morning and ending at noon on Sunday.<br />

During the quarters the diaconate formation<br />

program meets elsewhere, classes are held on the<br />

ten Saturdays of the quarter.<br />
