2. Except as otherwise provided herein, retirees who are eligible must select the full<br />

program, including both Health and Dental, or he/she will be considered to have waived<br />

his/her eligibility rights.<br />

3. A retiree who has attained the age of 60 years or older and who has coverage under<br />

his/her spouse’s plan shall be eligible for the contractual premium allowance as follows:<br />

a. By purchasing health coverage benefits within a spouse’s plan, beyond the benefits<br />

already provided, not to exceed the benefit levels in the District’s plan, the cost shall not<br />

exceed the premium cost the District pays for its own plan benefits. Where the primary<br />

health care provider will allow, the premium for such coverage shall be paid by the<br />

District directly to the carrier. In the event that the primary carrier will not accept such<br />

payment, the premium amount shall be paid directly to the retiree, subject to the issuance<br />

of a 1099 form at year end; and/or<br />

b. By purchasing dental benefits when such benefits are not included in a spouse’s plan,<br />

a retiree and/or his/her spouse may enroll in the District dental plan as an individual,<br />

provided that the District’s contribution for such coverage shall not exceed the contract<br />

premium allowance limit.<br />

4. A retiree who has attained the age of (See Appendix. E5) years or older and who<br />

maintains coverage in a non-District plan, other than that obtainable through a spouse,<br />

shall not be eligible for the District dental coverage, but will be eligible for the<br />

contractual premium allowance necessary to cover the cost of non-District health benefit,<br />

not to exceed the contract premium allowance limit. Where the primary health care<br />

provider will allow, the premium for such coverage shall be paid by the District directly<br />

to the carrier. In the event that the primary carrier will not accept such payment, the<br />

premium amount shall be paid directly to the retiree, subject to the issuance of a 1099<br />

form at year end.<br />

5. No retiree or his/her spouse shall be eligible for the contracted premium allowance prior<br />

to reaching the ages as stipulated in Appendix E5.<br />

6. Persons who retired prior to this agreement shall be eligible for the above health benefits,<br />

provided she/he meet the eligibility requirements as set forth herein.<br />
