Community Physical Disability Team (CPDT)<br />
Aims<br />
• To <strong>of</strong>fer individuals time limited treatment and support to enable<br />
people to live in the community as independent as possible<br />
• To <strong>of</strong>fer support, advice and guidance to carers or families<br />
• To work closely with other staff and agencies already involved<br />
supplementing their in put<br />
• To provide information and advice on community services available to<br />
people with physical disabilities<br />
Main Activities and <strong>Services</strong><br />
Help advice and specialist therapy in a variety <strong>of</strong> areas:<br />
• mobility • continuance<br />
• day to day activates • weight loss or gain<br />
• speech and communication • swallowing<br />
• skin breakdown • memory problems<br />
• mood problems<br />
