Momentum Community Solutions<br />
- Support and Rehabilitation Service<br />
Aims<br />
• Momentum’s community solutions is an individually tailored package<br />
to provide round the clock support for adults with brain injury. This<br />
enables service users to live independently in their own tenancy and<br />
located in their own community. The service operates on a 24 hour<br />
seven days a week basis<br />
