Eligibility<br />

beneficiaries OF DECEASED ORP RETIREES<br />

How You Will<br />

Pay for <strong>Benefits</strong><br />

You are eligible for health benefits coverage if you are the<br />

surviving spouse or child of a deceased ORP Retiree and:<br />

u You are receiving a periodic distribution of benefits<br />

from the Retiree’s Maryland ORP; and<br />

u You meet the spouse or child dependent eligibility<br />

criteria for health benefits.<br />

If the surviving spouse is the beneficiary, the spouse<br />

may cover himself/herself and any eligible dependent of<br />

the deceased ORP Retiree. However, only dependents<br />

that would be eligible dependents of the deceased ORP<br />

Retiree if he/she were still living may be covered.<br />

If a child is the beneficiary, only the child will be eligible<br />

for health benefits as long as he/she meets dependent<br />

eligibility requirements for children (see page 7).<br />

Required Documentation: Completion of a Retiree<br />

<strong>Health</strong> and Welfare <strong>Benefits</strong> Enrollment Form and<br />

a State of Maryland Optional Retirement Program<br />

(ORP) Packet. The form and packet are available from<br />

an Agency <strong>Benefits</strong> Coordinator, at a Maryland State<br />

institution of higher education, from our website,<br />

www.dbm.maryland.gov, or by mail from the Employee<br />

<strong>Benefits</strong> Division.<br />

u Maximum State subsidy if the<br />

Retiree had service equal to 25<br />

or more full years of regular<br />

employment with the State<br />

in any branch of government;<br />

you may be eligible for the<br />

maximum State subsidy even<br />

if the Retiree did not retire<br />

directly from a Maryland State<br />

institution of higher education.<br />

u No State Subsidy if the<br />

Retiree had less than 25<br />

years of Maryland State<br />

service.<br />

ORP Beneficiaries may cover<br />

dependents that would be eligible<br />

dependents of the deceased<br />

Retiree.<br />

Same as ORP Retirees (see page 10).<br />

NOTE: If all coverage in the <strong>Health</strong> <strong>Benefits</strong> Program is terminated for an ORP Retiree or Beneficiary for any reason, either<br />

voluntarily or involuntarily, documentation confirming the current continuous receipt of a periodic distribution from the<br />

Maryland ORP must be provided to qualify for re-enrollment.<br />
