INFORMATION FOR COMPLETING
2013 CONTRACTOR’S REGISTRATION APPLICATION
As prescribed by Chapter 747 of the Codified Ordinances of the City of Upper Arlington, all General Contractors, Building Trade Contractors and
Subcontractors are required to obtain a Contractor’s Registration prior to commencing work in the City.

Requirements for each Contractor are as follows:
General Contractors: 1. $5,000 ORIGINAL Bond
(Includes Signs & Swimming Pools) in favor of the City of Upper Arlington
with surety company’s affixed seal or
embossed seal valid through 1-1 to 12-31-13.
2. $300,000 Liability Insurance showing
the City of Upper Arlington and our address as the
Certificate Holder.***
3. $94.50 Registration Fee
Plumbing Contractors: 1. $5,000 ORIGINAL Bond
in favor of the City of Upper Arlington
with surety company’s affixed seal or
embossed seal valid through 1-1 to 12-31-13.
2. Current STATE OF OHIO, Department
of Commerce Certificate. Applicant must be the same
as certificate holder.
3. $94.50 Registration Fee
Sewer Builder and Driveway/
Sidewalk Contractors: 1. $5,000 ORIGINAL Bond
in favor of the City of Upper Arlington
with surety company’s affixed seal or
embossed seal valid 1-1 to 12-31-13.
2. $94.50 Registration Fee for each
type of registration.
Electrical, Refrigeration,
Warm Air Heating, Steam and
Hot Water, Ventilation/Air 1. Current STATE OF OHIO, Department
Conditioning Contractors:
of Commerce Certificate. Applicant must be the
same as the certificate holder.
2. $94.50 Registration Fee for each type
of registration.
House Moving Contractors: 1. $25,000 Bond
2. $94.50 Registration Fee
*** The General Contractor’s Certificate of Liability Insurance must be for a minimum combined bodily and property damage coverage in
the amount of $300,000, showing the City of Upper Arlington as a Certificate Holder and maintained in full force and effect through the
Registration Year. A copy of any policy changes, including renewal, shall be forwarded to the Building Division throughout the term of
the Registration.
