If the complaint involves an individual assignment grade or a final grade, the Dean may ask theinstructor to change the grade only in cases involving a procedural error in the conduct of the courseor due to evidence of unfair treatment.7. Upon receiptof the application, a degree audit will be prepared and sent to the student indicating clearance to graduate orwill list deficiencies that will prevent graduation as intended.Applications received after the first week of the final semester during which the student intends to graduateare subject to the following conditions:• Acceptance of the application after the deadline does not guarantee eligibility to graduate as intended.Eligibility to graduate is dependent upon the student meeting all degree requirements.• Applications received after the first week of the final semester during which a student intends tograduate will result in a late fee of $150 (payable in advance) regardless of his/her eligibility tograduate.• Acceptance of the application after the first week of the final semester during which the student intendsto graduate does not guarantee that applicants will receive tickets for Commencement even if weconfirm your eligibility to graduate. All planning, including ticket distribution numbers, are basedupon the number of students who apply to graduate by the deadline.All associate degrees require a minimum of 64 hours of which 24 of the last 30 hours must be completed atUIW. All baccalaureate degrees require a minimum of 128 hours of which 42 must be advanced. Aminimum grade point average of 2.0 is required to graduate. Academic Literacy and English as a SecondLanguage courses will not count toward either the required 128 hours or the 2.0 grade point average.Specific graduation requirements are listed in the University catalog. It is the student’s responsibility toensure that all University requirements have been met for graduation. To participate in graduationceremonies, a student must have registered for and remain enrolled in all required courses necessary forgraduation before or during the same semester in which she/he intends to graduate.Should a student not complete the course required for graduation under the application for the conferralperiod indicated by the student, s/he must reapply for graduation and pay applicable fees, to include thediploma reorder fee as well as any late fees. Diplomas will not be released until the student has met allUniversity obligations (financial or otherwise). Students may not participate in the Commencementceremonies unless their account balance is paid in full by the Friday of the week before finals begin.Probation and Suspension (Back to TOC)Full-time undergraduates who earn a grade point average of less than a 2.0 (C) in any 12 semester hours ormore will be placed on scholastic probation.44
A student on probation for one semester may choose to withdraw or may be continued on probation for oneadditional semester. If, at the end of this semester, the student has not been removed from probation, uponthe recommendation of the Committee on Student Academic Probation and Suspension, the student will berequired to withdraw from the University. Part-time students enrolled for at least six semester credits andearning less than a 2.0 may be placed on probation. Part-time students who fail to earn a C (2.0) in any sixhours out of 12 semester hours attempted may be required to withdraw from the UniversityPolicy on Privacy of Student Records (Back to TOC)The University of the Incarnate Word maintains educational records for all current and former students whoare officially enrolled. Student records at the University are subject to the provisions of the FamilyEducational Rights and Privacy Act of 1974, as amended. Students have the right to file a complaint withthe U.S. Department of Education concerning compliance issues. The name and address of the appropriateoffice is:Family Policy Compliance OfficeU.S. Department of Education400 Maryland Avenue, SWWashington, DC 20202-4605Review of the Educational RecordStudents have the right to inspect and review their educational record. All requests must be in writing to theRegistrar and must identify the record(s) they wish to inspect. The University official will makearrangements for access and notify the students of the time and place where the records may be inspected.If the University official to whom the request was submitted does not maintain the records, that officialshall advise the student of the correct official to whom the request should be addressed. The response fromthe University official will be within 45 days of the receipt of the request for access.Educational Records on CampusEducational records are those records directly related to a student for the purpose of recording theeducational endeavor of the student. They do not include law enforcement records, employment records,medical records, alumni records or faculty advisor/instructor notes. Educational records may be stored inmany media and are not limited to an individual file. See Table III.Amendment of the Educational RecordThe student may request an amendment to the educational record if she/he believes it is inaccurate ormisleading. The amendment of the educational record does not pertain to the grade assigned by the faculty.The student should write to the Registrar to request the amendment. The request must clearly identify theportion of the record she/he wants changed, specifying why it is inaccurate or misleading. If the Universitydoes not amend the record as requested, the University will notify the student of the decision and advise thestudent of his or her right to a hearing regarding the request for amendment. Information regarding thehearing procedures will be provided to the student when notified of the right to a hearing.Directory InformationThe University of the Incarnate Word will not disclose any personally identifiable information aboutstudents (except directory information listed below) without the written consent of the student.Directory information at the University of the Incarnate Word has been identified as:1.
