IHE Patient Care Device Technical Framework Supplement – Alarm CommunicationManagement (ACM)______________________________________________________________________________6456506556603.Y+1.4.1.3 HL7 Conformance StatementThe conformance statement for the interaction described below is adapted from HL7 2.6.While HL7 2.8 item 625 provides for the Alarm Manager (AM) to send either ORA^R41 orORA^R42 as Report Alarm Status [PCD-05] to the Alarm Reporter (AR), ORA^R41 as definedin HL7 is not expected to be utilized by vendors as it presumes a guarantee of delivery that theAlarm Manager (AM) and the Alarm Communicator (AC) cannot assure. However, since theIPEC profile uses the R42 event which was originally intended for use by ACM, the ACMprofile will use R41 to mean what R42 was originally meant to convey, that being the alarmcommunication status from the AM back to the ARThe AM and AC cannot provide any level of assurance of delivery of the alarm to the endpointdevice. This lack of assurance of delivery is based upon the predominant use by healthcare ofcost conscious one-way fire-and-forget pagers.The PCD-04 message will presume a default filter for PCD-05 notifications so that only“delivered to one or more recipients” or “not successfully delivered to any recipients” will be theonly notifications that the AR actor has to handle. This avoids the AM back to AR fire hose forall device specific notification activity details for all devices to which a notification is delivered.This also avoids the requirement for a filter segment in the PCD-04 message.
