Messagefrom the Executive DirectorDear Colleagues and Friends,It is a great privilege to lead <strong>MEDiCAM</strong>, amembership organization for NGOs working inCambodia’s Health Sector, with 128 local and InternationalNGO members.Despite the many challenges from the verybeginning, the year <strong>2011</strong> has ended with continuedgood enabling environment—(1) the Prime Ministerof the Royal Government of Cambodia officially declaredthe delay of the Association and NGO Lawuntil both government and CSOs reach consensus ofthe content of the law, and (2) the Busan OutcomePrinciples on Development Effectiveness have enforcedthe inclusiveness of key pillars of a society(government, private sector, CSOs, and community)as the driving forces for sustainable and equitablegrowth and development.<strong>MEDiCAM</strong> is proud to be part of this activejoint advocacy efforts on the NGO law and the developmenteffectiveness—the Siem Reap Consensusleading to Busan, and the Busan Outcome Principles.Letting the world impact us is NOT ourchoice. The world impacts our daily lives tremendously—theworse or the better. Impacting the worldis a must. To bring country’s experience and evidenceto somehow impact global policies, <strong>MEDiCAM</strong> isan active member of the World Bank CSO ConsultativeGroup for Health, Nutrition, and Population(CSCG HNP) and a CSO focal point at country levelfor International Health Partnership.The CDHS 2010 indicated remarkableachievements, making Cambodia among a few countriesreach MDGs 4, 5 and 6 in 2015. The NGO communityand <strong>MEDiCAM</strong> have obviously contributedsignificantly to these amazing successes of theHealth Sector.<strong>MEDiCAM</strong>’s <strong>Annual</strong> <strong>Report</strong> <strong>2011</strong> has highlightedthe details of what exactly <strong>MEDiCAM</strong> did in itsparticipation as well as to enable the participation ofhealth NGOs in improving health and strengtheninghealth system in Cambodia. It is very much recommendedto look at in details in the areas including (1)promotion of evidence-informed policy development,advocacy and representation, (2) Coordination,Harmonization andAlignment, (3) Social Accountabilityand GoodGovernance, (4) CapacityBuilding, and (5) informationsharing.Through this reportyou will see that <strong>MEDiCAM</strong>has been working not onlyadvocacy at higher policyDr. Sin Somunylevel but also mobilizing local authorities and theaffected communities to advocate for better healthand health system at the operational level. On the onehand, <strong>MEDiCAM</strong> is working to ensure that theNational Policies/strategies and plans reflect thecountry’s and community’s needs. And on the otherhand, <strong>MEDiCAM</strong> is working with our NGO membersto ensure that their technical and financialassistance are harmonized and aligned with thoseNational Policies/Strategies/and Plans.Taking this opportunity, on behalf of staff,the management, and the Steering Committee of<strong>MEDiCAM</strong>, I would like to express my sincerethanks to the Royal Government of Cambodia andthe Ministry of Health for providing enabling environment,the support and partnership. The studentsfrom International University were the opponents to the ideas. Evidence›Informed Policy Development, Representation and AdvocacyRoyalGovernmentof CambodiaNGOsHealth Care Financing Debate with participation from the National Assembly and Senateon 23 rd November with the support from IHP and GFATM-HSSOn 23 rd November, <strong>2011</strong>, theThink Tank in collaboration with theMinistry of Health, Department ofPlanning and Health Information,organized a policy debate on health carefinancing. Dr. Robert Yates, HealthEconomist, WHO’s Directorate onHealth Care Financing, Geneva, was thedistinguished speaker together with Dr.Lo Veasna Kiry, Director of Departmentof Planning and Health Information,Ministry of Health (MoH). He presented the WHO’s 2010 report on Health CareFinancing with emphasis on reaching universal coverage More interestingly, Dr. Robertpresented how countries with similar socio-economic context to Cambodia are now onthe way to reach Universal Coverage and best examples of several other countries in theregion such as Thailand that uses Tax-Based Financing System to reach universalcoverage of quality health care to its population. The chair of Commission number 8 ofthe National Assembly, Lok Chum Teav Ho Non, the Deputy Chair of the 8 thCommission of the Senate, Lok Chum Teav Pum Sichan, and several otherparliamentarians actively attended the debate. RoyalGovernmentof CambodiaNGOsEvidence›Informed Policy Development, Representation and AdvOther Important Advocacy Efforts in <strong>2011</strong><strong>MEDiCAM</strong> has consolidated the voice of NGOs into a <strong>MEDiCAM</strong> position paper<strong>2011</strong> in which all three priorities and five cross-cutting pillar strategies of HealthStrategic Plan have been reviewed. <strong>MEDiCAM</strong> Position paper has highlightedkey issues, recommendations, and how NGOs can contribute to jointly address thechallenges. Among global CSOs as members of the WBCSO Consultative Group for Health, Nutrition and Population, <strong>MEDiCAM</strong> is amember of HSS Task Force and Health Care Financing Task Force, writingposition paper on those two topics to influence the WB policy on HSS and HealthCare Financing in its coming <strong>Annual</strong> Meeting in 2012 in Tokyo.Three NGO good practices have been collected—(1) Emergency Obstetric andNewborn Care, (2) Safe Abortion, and (3) Home-based care. All these documentsare available at our website.<strong>MEDiCAM</strong> in partnership with VSO has been conducting a qualitative researchon understanding health worker motivation in five regions of Cambodia (Ministryof Health’s Regions)—Stung Treng, Bantey Meanchey, Kampong Cham, Takeo,and Phnom Penh. The objectives of the research are to come to a betterunderstanding of why health workers adopt behaviors that impact negatively onpatients and to look for solutions in policy and practice to improve staffmotivation and morale. It is expected that the research will provide valuableevidence for policy change dialogue with government and donor levelstakeholders and capacity building with NGOs and health workers within thesector. For year <strong>2011</strong>, the field data collection has completed within 4 regions. Itis expected that the preliminary results will be in April 2012 and the final reportwill be probably in June 2012.<strong>MEDiCAM</strong> in partnership with Center of Health Market Innovation (CHMI) inVietnam with the support from Results for Development Institute (R4D)conducted studies on identifying Health Market Innovations which have beenimplemented in Cambodia by government, Development Partners, Private Sectoror NGOs. In <strong>2011</strong>, <strong>MEDiCAM</strong> has collected 53 project profiles with potentialHealth Market Innovations. Of which, 6 cases were selected for documents ofHealth Market Innovation. These include (1) Fast Track Initiative for reducingmaternal and newborn care, (2) Lake Clinic program on Tonle Sap, (3) HealthEquity Fund managed by pagoda, (4) Special Operation Agency (SOA), (5) SmartPhone, and (6) Oral Sel. Community Advocacy Champion PlansThis tool was adopted toencourage and build confidence ofmarginalized communities who aremost often neglected and not part ofany decision-making processes, toovercome their fear and becomeeffective advocates and spokespersonfor their communities.There were fourteencommunity advocacy champion plansdeveloped and implemented during thereport period. Six advocacy championplans were developed andimplemented in Kirivong OD, Takeoprovince; four advocacy championplans in SmachMeanchey OD, KohKong province; and fouradvocacychampion plans in Sre AmbelOD, Koh Kong province. Additional48 community Advocacy workplans in4 ODs of Siem Reap, Battambang,BTB and PVH are being implementedand expect to get the result ofadvocacy at the end of February 2012,The 48 work plans were funded byGF while 14 champion plans in ODKirivong, Smach Mean Chey and Sreambel funded by EUD. Impacts and Successful Stories of CHFs, and CHDs,There were three technical methods to follow up the commitments of health providers madeduring the Community Health Forum (CHF). Those included (1) Community Health Dialogue(CHD), (2) Interview with local NGO partners, and (3) interview with patients. The impactsand successful stories from are listed below:1. Behaviors of health providers toward their clients have been improved, especiallyat SmachMeanchey ,ThmorSar, Trapaing Rung and Chipat HCs in Koh KongProvince and in Battambang Provincial Hospital. (I am Home Best Care Teamleader in Prey Snao village. I would like inform that RH has improved a lot in thelast one year. Anonymous boxes were established to collect feedbacks on quality of care andbehaviors of health staff, for instance three anonymous boxes were set up atSmachMeanchey, two at Sre Ambel RH.3. The communication between health care providers and clients was improved,especially at Kirivong , Smach Mean Chey, Sre Ambel referral hospitals, atPrasatBakongMeanchey HC in Siem Reap Province and at Phnom Dek HC inPreahVihear Province. Health care providers have changed their behavior andpatients have changed from complaints to thanking health providers.4. Increased utilization of health services at public health facilities (Kirivong,SmachMeanchey ,Sre Ambel, Battambang RH, and 16 January RH in preahVihearprovince).5. The ART program of SmachMeanchey RH in Koh Kong province was improved6. Free transportation for corpse from SmachMeanchey RH to pagodas was providedby the Hospital.In Battambang, the poor families were also provided freetransportation for bringing corpse back to their homeland or pagoda. This can bedone with approval from the RH Management Committee.7. Health care staff respects working hours and health providers now standby 24hours a day and 7 days a week at emergency room of SmachMeancheyRH and inAnkorthom HC in Siem Reap province.8. There is no more extra charge at the maternity ward of SmachMeanchey referralhospital and in Battambang RH9. The proposal of connecting water for Sre Ambel RH was approved by Districtoffice of environment10. PLHAs in Sangke OD of Battambang province have received free health careservices at Health Centers since 07 September <strong>2011</strong> and11. Patients who are holding ID-Poor in Sangke OD of Battambang province stoppedpaying extra money at Battambang Referral Hospital since 07 September <strong>2011</strong>.12. In Battambang RH, patients who needed blood transfusion had to find the bloodfrom their family members to replace the blood in stock because it is to balance ofblood in stock. Normally, poor family and PLHIVs were not able to get blood. The Success of Six Community Advocacy Champion Plans in Kirivong OD,Takeo Province1) The first advocacy champion plan implemented in Kirivong OD was “OVCs canaccess to free health care services at Rominh, Krapum Chhouk, and PreyMelong HCs”. The OVCs living under coverage areas of three HCs were listedby champion group in collaboration with local authorities and then championgroup brought those numbers of OVCs to advocate with three HCs above. Asresults, 59 OVCs (30 female) were approved to receive free health care service inwhich 31 OVCs (15female) at Rominh HC, 23 OVCs (11.female) at KrapumChhouk HC, and 5 OVCs (4female) at Prey Melong HC.2) The second advocacy champion plan implemented in Kirivong OD was “Poorwidow can access to free health care services at Chi Khmar, Prambey Mum,and Samlong HCs”. 59 poor widows were approved to receive free health careservices in which 23 people at Chi Khmar HC, 22 people at Samlong HC, and 14people at Prampey Mum HC.3) The third advocacy champion plan carried-out in Kirivong OD was “PoorDisable People can access to free health care services at Angknor,DoungKpos and Kork Pour HCs”. 18 Poor PwDs (6 female) were approved toreceive free health services in which 13 people (3 female) at Angknor, 2 people (2female) at DoungKpos, and 2 people (1 female) Kork Pour HC.4) The fourth advocacy champion plan carried-out in Kirivong OD was “Poorelderly people can access to free health care services at Choun Chum, AngPrasat, Kamnub, and Phnom Den HCs”. 89 poor elderly people (48 female)were approved to receive free health services in which 59 people (22 female) atChoun Chum HC, 15 people (12 female) at Ang Prasat HC, 9 people (8 female) atKamnub HC, and 6 people (6 female) at Phnom Den HC.5) The next advocacy champion plan carried-out in Kirivong OD was “OVCs canaccess to free health care services at KoakPrich and Kampeng HCs”. 60OVCs (22 female) were approved to receive free health services in which 14OVCs (4 female) at Kampeng HC and 46 OVCs (18 female) at KoakPrich HC.6) The last advocacy champion plan carried-out in Kirivong OD was “Poor disablepeople can access to free health care services at Som and Prey Romdeng HCsand Ta-O HP”. 47 Poor PwDs (25 female) were approved to receive free healthcare services in which 18 people (4 female) at Prey Romdeng HC, 17 people (10female) at Som HC, and 13 people (7 female) at Ta-O HP.F. Four Community Advocacy Champion Plans in SmachMeanchey OD, Koh KongProvince1) The first advocacy champion plan carried-out in SmachMeanchey OD was“Poor disable people can access to free health care services atSmachMeanchey RH”. RoyalGovernmentof CambodiaNGOs4CapacityBuildingCapacity Building<strong>MEDiCAM</strong> Capacity Building Program, with the support from the Global Fund SSF HSS and SSF HIVand EU, organized 25 training courses in <strong>2011</strong>. These trainings covered all NGOs working in the entirecountry.<strong>MEDiCAM</strong> training focuses on organizational development, health related topics, and advocacy. Whatmakes <strong>MEDiCAM</strong> training unique is that <strong>MEDiCAM</strong> provides training, coaching, and giving smallaward for grassroots NGOs together with community and local authority to develop their plans forimplementation. We have done this for Communicating for advocacy. RoyalGovernmentof CambodiaNGOsCapacity BuildingTraining ImpactHaving conducted the training following-up to understand the impact of the training among a randomizedselection of sample size of our trainees, <strong>MEDiCAM</strong> has found the following results: 100% replied that they have used the skills they learnt to improve their work.The skills which they specifically used include staff evaluation, staff motivation, writing jobdescription, design project, monitoring and evaluation, staff recruitment, Logical frame workdevelopment, problem analysis, objective analysis, health education skill, facilitation skill, roleof HCMC/VHSG, using IEC materials, teaching method, communication skill, Health system inCambodia, small survey, Epidata , and revise strategic plan and policies. 75% of respondents said they have shared their knowledge and skills with others. Usually theyshared the new skills during the staff meeting or quarterly meeting. They shared this knowledgeby chatting, telling, reporting and giving them documents to read. Sometimes they preparedPowerPoint presentation on specific topic to share with their colleagues.100% reported that they feel the new skills helped to improve the effectiveness of their work.They said that they can and have full confidence to provide education to people. They are able touse IEC materials for training. They can develop action plan and strategic plan for theirorganization. They know how to refer patient through link response, and they know how tocommunicate with people. They can develop simple database for data entry and analyze data.Furthermore, they told us that they now have much better understanding of health system andnational policy.80% reported that the level knowledge acquired during the training is adequate for their job. It'snot only these skills or knowledge that they need to apply in their project areas but there aremany other skills/knowledge which are also necessary for them to have in order to improve theeffectiveness in their project. Capacity BuildingRoyalGovernmentof CambodiaNGOs80% of all line managers asked reported that the training their staff received was the right topicand the right skill which are relevant to their daily work and the acquired skills have been used toimprove the work of the organization as a whole.Most of line managers asked told us that they discussed with their staff how they experienced thetraining and they felt that the training has made them more confident in performing her presenttask. Some of them said that their staffs were able to develop training curriculum and preparelesson plan for the training. They can mobilize community to participate in the projectimplementation, monitoring and evaluation. One manager said that <strong>MEDiCAM</strong> trainings are better than other organizations’. 20% of line managers said they have given their staff more responsibilities after the training.These tasks include preparing lesson plans, IEC materials and other educational messages. Theyalso encouraged their staff to more regularly attend other NGOs meetings such as <strong>MEDiCAM</strong>'s inorder to update themselves with latest information in the health sector.In conclusion, participants had a chance to implement key learning in their work after training and thenew skills to improve effectiveness of their work. They became more self-confident to provide educationto people and understand health system and policy. The level of knowledge/skills acquired during thetraining is useful for their jobs.The <strong>MEDiCAM</strong>’s training is very effective for participants. All respondents said that they had a chanceto use the skills acquired to improve themselves in their professionalism, improve the work of theirorganization, and <strong>MEDiCAM</strong> has also helped strengthening their institutional development capacity.Success Stories:Translating skill into concrete actions and outcomes:Case 1: Strategic Planning, a building management capacity of Cambodian’s NGOs which havedeveloped their own strategic plan in order to achieve their vision, mission, goalStrategic plan development has been an uphill challenge for many grassroots or community-based NGOsor even several local NGOs.<strong>MEDiCAM</strong> has started this project since 2006 with the support from Canadian InternationalDevelopment Agency (CIDA). The support from CIDA for two years has built <strong>MEDiCAM</strong> with goodcapacity to extend this program through the GFATM SSF HIV/AIDS.With this support, the strategic plan training was conducted in Battambang province from 17 to 21October, <strong>2011</strong>. In December <strong>2011</strong>, in our coaching visit with Indigenous’ People Health ImprovementAssociation (IPHIA), the trainees from this organization with further assistance from <strong>MEDiCAM</strong>coaching visit of the Organizational Development Trainer/Coach, IPHIA has developed its first strategicplan <strong>2011</strong>-2020.The director of this organization has expressed their profound gratitude to <strong>MEDiCAM</strong> and the supportfrom the GFATM that has made them able to have a better strategic direction which helps theorganizations to work more strategically and this could lead to more effectiveness and efficiency inspending our resources. In January 2012, in our follow up training with Adventist Development andRelief Agency (ADRA) and Cambodian Women … (CWCC), the trainees from these organizations havesuccessfully designed project and supported from their donors.The director of this organization has expressed their profound gratitude to <strong>MEDiCAM</strong> and the supportfrom the GFATM SSF HIV/AIDS that has made them able to have a better project design andmanagement which helps their staff to work and manage more effectiveness and efficiency in spendingour resources.RecommendationsThe results of training follow-up suggest that <strong>MEDiCAM</strong> should early announce to member about theavailability of training and invite guest speaker from other organization or institution for some topics.The homogeneity of participant should be considered and refresher course should be conducted everyyear.<strong>MEDiCAM</strong> reviewed and upgraded the training contents to intermediate level and candidates only beselected when they fit the selection criteria. RoyalGovernmentof CambodiaNGOsCapacity BuildingCommunity Advocacy Work Plan Implementation and AchievementsWith the support from the GFATM-HSS, <strong>MEDiCAM</strong>’s program is not only about providing training on“communicating for advocacy” but also support the vulnerable community and local authority to develop“Community Advocacy Work Plan”.In year <strong>2011</strong>, 96 Community Advocacy Work Plans have been developed after the rolling out training.Out of this figure, 48 Community Advocacy Work Plans were endorsed by the Community AdvocacyWork Plan Certification Committee (CAWPCC), composed of OD director, commune council, grassrootsNGO, SSR of <strong>MEDiCAM</strong>, <strong>MEDiCAM</strong> officer.Three Example of Success Stories from the implementation of these Community Advocacy WorkPlans:Friend Without a Border (FWAB) is an SSR of <strong>MEDiCAM</strong> implementing the GFATM-supported HSS. Inworking with the vulnerable communities, commune councils, FWAB was able to give an example of thesuccesses of the implementation of the Community Advocacy Work Plans.The members of the Community Advocacy Work Plan in Kandeck, Preah Dak, and Prasat Bakong HealthCenters have found that there were 41, 31, and 21 drug sellers, respectively in their catchment areas ofthose health centers. The meeting was chaired by OD director, andparticipated by local authority, HCMC, VHSGs, health center staff, and NGO representatives. The mainagenda of the meeting was on the issue of unlicensed drug sellers in these above mentioned communes.The meeting unanimously decided to eliminate unlicensed drug sellers within their communities. Thereport of the meeting was sent to all relevant local authorities to take immediate actions.OD and local authorities (Commune Councils and District Councils) took action by inviting all drugsellers in their communities to hold a meeting, explaining the Ministry of Health’ s regulation oneliminating all unlicensed drug sellers and the negative impact on our communities’ health. All drugsellers agreed to sign an agreement to stop selling their drugs until proper registration and licensing isgranted. In preparingthese statements, which gives a true and fair view, it is fundamental that appropriate accountingpolicies are selected and applied consistently. Our responsibility is to express an opinion on thefinancial statements based on our audit.Basis of opinionWe conducted our audit in accordance with international standards on auditing. An audit includesexamining, on a test basis, evidence supporting the amounts and disclosures in the balance sheetsand statements of revenue and expenses.
