The City of Champaign reserves the right <strong>to</strong> accept or reject any or all <strong>proposals</strong>,or portions thereof.D. When a package proposal is made for a package price, the package price <strong>shall</strong> <strong>be</strong>taken <strong>to</strong> <strong>in</strong>clude all items referenced <strong>in</strong> the package.E. The Vendor may submit multiple <strong>proposals</strong> <strong>in</strong>volv<strong>in</strong>g various methods of meet<strong>in</strong>gproposal objectives. However, each <strong>submitted</strong> proposal <strong>shall</strong> <strong>be</strong> complete <strong>in</strong>every respect and marked as Proposal No. 1, No. 2, etc. on the cover page.F. G. Section and subsection num<strong>be</strong>r<strong>in</strong>g should <strong>be</strong> <strong>in</strong> a consistent format us<strong>in</strong>g thenum<strong>be</strong>rs <strong>in</strong>dicated <strong>in</strong> the “Content and Format” section of the <strong>Request</strong> and<strong>be</strong>g<strong>in</strong>n<strong>in</strong>g each section on a new page.1.4 Content and FormatAn orig<strong>in</strong>al plus the required num<strong>be</strong>r of copies of the proposal <strong>shall</strong> <strong>be</strong> <strong>submitted</strong> along with allthe required documentation. The orig<strong>in</strong>al and copies of the proposal <strong>shall</strong> <strong>be</strong> packagedseparately and securely bound and sealed. Each <strong>shall</strong> show the name and address of theVendor submitt<strong>in</strong>g the proposal on the front cover.Proposals should <strong>in</strong>clude the follow<strong>in</strong>g <strong>in</strong> the order listed. Each section should <strong>be</strong> la<strong>be</strong>led.a. Cover LetterRespondents <strong>shall</strong> provide a cover letter or letter of transmittal that briefly summarizesthe company's <strong>in</strong>terest and relevant qualifications for the project. This letter <strong>shall</strong> notexceed two (2) pages, and <strong>shall</strong> <strong>be</strong> signed by an agent of the respond<strong>in</strong>g firm who isauthorized <strong>to</strong> negotiate the details of the proposed services.b. Vendor Information and Executive SummaryThe Executive Summary is limited <strong>to</strong> five (5) pages and should <strong>in</strong>clude a writtensummary of the proposal for review and, at a m<strong>in</strong>imum, the follow<strong>in</strong>g <strong>in</strong>formation:1. 1.8 Cus<strong>to</strong>mer Service(4) The previous and exist<strong>in</strong>g compliance by the Vendor with laws andord<strong>in</strong>ances relat<strong>in</strong>g <strong>to</strong> the contract;(5) The quality, availability and adaptability of the supplies, mach<strong>in</strong>ery, plan<strong>to</strong>r other equipment or contractual services <strong>to</strong> the particular use required;(6) The sufficiency of the f<strong>in</strong>ancial resources and f<strong>in</strong>ancial ability of theVendor <strong>to</strong> enter <strong>in</strong><strong>to</strong> and perform the contract or service;(7) Whether the Vendor is entitled <strong>to</strong> Local Preference pursuant <strong>to</strong> Section12.5-34 of the Champaign Municipal Code, 1985 as amended;(8) Completion and approval of the EEO/AA paperwork.The City expects the Vendor <strong>to</strong> deliver a high level of cus<strong>to</strong>mer service regard<strong>in</strong>g thema<strong>in</strong>tenance, repair, delivery, and <strong>in</strong>stallation of any equipment and the provision of anyservice <strong>to</strong> the City, its employees and its cus<strong>to</strong>mers.1.9 Rights <strong>to</strong> Submitted Materials<strong>All</strong> <strong>proposals</strong>, <strong>response</strong>s, <strong>in</strong>quiries, or correspondence relat<strong>in</strong>g <strong>to</strong> or <strong>in</strong> reference <strong>to</strong> <strong>this</strong><strong>Request</strong>, and all reports, charts, displays, and other documentation <strong>submitted</strong> by the Vendor<strong>shall</strong> <strong>be</strong>come the property of the City when received and <strong>shall</strong> not <strong>be</strong> returned <strong>to</strong> the Vendor.The City reserves the right <strong>to</strong> use the material or any ideas <strong>submitted</strong> <strong>in</strong> <strong>this</strong> proposal <strong>in</strong><strong>response</strong> <strong>to</strong> the <strong>Request</strong> whether amended or not. Selection or rejection of any proposal doesnot affect <strong>this</strong> right.1.10 Proprietary InformationAny restrictions on the use of <strong>in</strong>formation conta<strong>in</strong>ed with<strong>in</strong> a proposal <strong>shall</strong> <strong>be</strong> clearly stated assuch with<strong>in</strong> the proposal. The City will only <strong>be</strong> able <strong>to</strong> comply with a request for confidentiality<strong>to</strong> the extent allowed by law.1.11 Affirmative ActionIf <strong>this</strong> contract will <strong>be</strong> over $17,500, the Vendor <strong>shall</strong> comply with the Equal Opportunity <strong>in</strong>Purchas<strong>in</strong>g Ord<strong>in</strong>ance of the City of Champaign (Section 12.5-65 of the Champaign MunicipalCode, 1985, as amended). Pursuant <strong>to</strong> the Ord<strong>in</strong>ance, the Vendor must have on file aCertificate of Approval or an Affirmative Action Form plus an approved Affirmative Action Plan<strong>be</strong>fore a Vendor can <strong>be</strong> selected. 1.12 Acceptance of Proposal ContentThe contents of the proposal or parts thereof selected by the City will <strong>be</strong> <strong>in</strong>corporated <strong>in</strong><strong>to</strong> thef<strong>in</strong>al Contract Documents <strong>to</strong> the extent they comply with <strong>this</strong> <strong>Request</strong>.1.13 Cost of the Vendor <strong>to</strong> Respond <strong>to</strong> <strong>Request</strong>The City is not responsible for any cost <strong>in</strong>curred by a Vendor <strong>in</strong> the process of respond<strong>in</strong>g <strong>to</strong> <strong>this</strong><strong>Request</strong> or for any pre-contract costs <strong>in</strong>curred by any Vendor participat<strong>in</strong>g <strong>in</strong> the selectionprocess.1.14 Public Advertis<strong>in</strong>gThe Vendor is specifically denied the right <strong>to</strong> use the name of the City of Champaign or Urbana-Champaign Big Broadband (UC2B) for public advertis<strong>in</strong>g or reference <strong>in</strong> any form or mediumwithout the express written permission of the City of Champaign.1.15 Term<strong>in</strong>ationThe City reserves the right <strong>to</strong> term<strong>in</strong>ate the selection process at any time, <strong>to</strong> reject any or all<strong>proposals</strong> and <strong>to</strong> award the contract <strong>in</strong> the <strong>be</strong>st <strong>in</strong>terest of the City of Champaign.9Form A-1 10/09


II.Scheduled Ma<strong>in</strong>tenance Services and Adds, Moves and Changes for FTTP ElectronicEquipment and Associated HardwareVendor may <strong>be</strong> required <strong>to</strong> update software or firmware for ONT’s, Gateway Routers andWireless Access Po<strong>in</strong>ts on a non-priority, scheduled ma<strong>in</strong>tenance basis. Software and/orfirmware updates will <strong>be</strong> <strong>in</strong>stalled on a scheduled service visit as updates are released. Vendormay need <strong>to</strong> respond <strong>to</strong> non-service effect<strong>in</strong>g alarm notices on a non-priority, scheduledma<strong>in</strong>tenance basis. <strong>All</strong> devices will receive security patches and firmware upgrades that will <strong>be</strong><strong>in</strong>stalled on a scheduled service visit as they are released.III.Local Phone Num<strong>be</strong>r for DispatchVendor will <strong>be</strong> required <strong>to</strong> provide local phone num<strong>be</strong>r for UC2B’s call center <strong>to</strong> dispatch repairand ma<strong>in</strong>tenance services.IV.Hours of Operations and Call VolumesThe Vendor <strong>shall</strong> <strong>be</strong> available <strong>to</strong> receive calls for trouble resolution, repair and ma<strong>in</strong>tenanceservices <strong>be</strong>tween the hours of 7:00 a.m. and 7:00 p.m. Monday through Friday. It is not yetknown what the peak volume period will <strong>be</strong> or how many calls are anticipated. The <strong>in</strong>itial grantis for approximately 2500 residential and bus<strong>in</strong>ess cus<strong>to</strong>mers and approximately 200 anchor<strong>in</strong>stitutions <strong>to</strong> <strong>be</strong> served. It is anticipated that <strong>this</strong> will <strong>be</strong> the <strong>to</strong>tal num<strong>be</strong>r of cus<strong>to</strong>mers <strong>in</strong> theprogram for the first year of the contract with the Vendor. After the <strong>in</strong>itial year, it is not yetknown whether or not the UC2B network will <strong>be</strong> extended <strong>to</strong> serve a larger footpr<strong>in</strong>t of theUrbana-Champaign area or whether additional bus<strong>in</strong>esses and commercial cus<strong>to</strong>mers will <strong>be</strong>served.V. Close out of Trouble TicketThe Vendor will <strong>be</strong> responsible for clos<strong>in</strong>g out any trouble tickets with the UC2B call center.VI.Replacement of EquipmentIf the ADTRAN, Gateway Routers or Wireless Access Po<strong>in</strong>t equipment needs <strong>to</strong> <strong>be</strong> replaced, theVendor will have a supply of replacement equipment from UC2B that will <strong>be</strong> replenished overtime as needed.VII.Material and Product ProtectionThe Vendor <strong>shall</strong> <strong>be</strong> responsible for protect<strong>in</strong>g f<strong>in</strong>ished work, equipment, supplies, andmaterials from loss, <strong>in</strong>jury, and/or damages from any cause whatsoever until f<strong>in</strong>al acceptanceor close-out of the trouble ticket or ma<strong>in</strong>tenance ticket by the appropriate UC2Brepresentative. Vendor <strong>shall</strong> <strong>be</strong> responsible for the proper care of any UC2B furnished11Form A-1 10/09


materials, supplies, and equipment while it is <strong>in</strong> their cus<strong>to</strong>dy. Vendor <strong>shall</strong> issue writtenreceipts for all such property and account <strong>to</strong> UC2B for any damage <strong>to</strong>, or loss of, such propertywhile <strong>in</strong> their cus<strong>to</strong>dy or control.VIII.Access <strong>to</strong> FacilitiesVendor <strong>shall</strong> coord<strong>in</strong>ate activities with the appropriate build<strong>in</strong>g owner, manager or tenant <strong>to</strong>ga<strong>in</strong> the required access. Vendors <strong>shall</strong> provide a (2) hour arrival w<strong>in</strong>dow for cus<strong>to</strong>mer’s repairor ma<strong>in</strong>tenance. Vendor <strong>shall</strong> also provide sufficient resources <strong>to</strong> coord<strong>in</strong>ate all necessaryaccess.IX.SafetyVendor <strong>shall</strong> adhere <strong>to</strong> all applicable federal, state, and local safety regulations and alloperations <strong>shall</strong> <strong>be</strong> conducted <strong>in</strong> a safe manner <strong>to</strong> protect both the public and the employees ofthe Vendor.X. Questions and ResponsesProvide <strong>in</strong> these Sections discussions on your technical qualifications and experience with repairand ma<strong>in</strong>tenance projects as related <strong>to</strong> the requirements above. Also, provide answers <strong>to</strong> thefollow<strong>in</strong>g questions.A. Previous management responsibilities related <strong>to</strong> the repair and ma<strong>in</strong>tenance of FTTPelectronic equipment and associated hardware of a full scale FTTP or similar project.Provide <strong>in</strong>formation regard<strong>in</strong>g your company’s current and past contracts, provid<strong>in</strong>gexperience <strong>in</strong> terms of similar size, scope, complexity and duration of projects.B. Please descri<strong>be</strong> your company’s approach <strong>to</strong> repair and ma<strong>in</strong>tenance services,describ<strong>in</strong>g <strong>in</strong> detail your trouble and ma<strong>in</strong>tenance ticket process, assignment oftechnicians, and trouble resolution process. How will your company <strong>in</strong>terface withUC2B’s Call Center? Descri<strong>be</strong> your approach <strong>in</strong> work<strong>in</strong>g with UC2B.C. Descri<strong>be</strong> the plan and personnel <strong>to</strong> perform repair and ma<strong>in</strong>tenance services andproject oversight of the work. Designate the key contact for UC2B <strong>to</strong> contact for anyproblem resolution. Descri<strong>be</strong> the plan <strong>to</strong> deal with escalated issues and the approach <strong>to</strong>solv<strong>in</strong>g issues. How many <strong>in</strong>dividuals with<strong>in</strong> your company are available and certified forrepair and ma<strong>in</strong>tenance services? Because of the nature of the work, it is required thatall Vendors’ personnel work<strong>in</strong>g on the project will <strong>be</strong> required <strong>to</strong> have a crim<strong>in</strong>albackground check and a pre-employment drug screen.D. Provide your safety record with OSHA <strong>in</strong>cident rates and their approach <strong>to</strong> safetytra<strong>in</strong><strong>in</strong>g. paperwork activity and report<strong>in</strong>g process, and other related project managementmethods.E. List and descri<strong>be</strong> any subcontrac<strong>to</strong>rs and def<strong>in</strong>e the role of the subcontrac<strong>to</strong>r <strong>in</strong>perform<strong>in</strong>g the work and the plan <strong>to</strong> manage any subcontrac<strong>to</strong>rs. Def<strong>in</strong>e the percentageof work <strong>to</strong> <strong>be</strong> assigned <strong>to</strong> the subcontrac<strong>to</strong>rs.F. If your company was <strong>to</strong> provide materials management and <strong>in</strong>ven<strong>to</strong>ry control,descri<strong>be</strong> <strong>in</strong> detail how you would perform <strong>this</strong>? Descri<strong>be</strong> the methods <strong>to</strong> track andreport materials and handl<strong>in</strong>g procedures. Indicate if there are any limitations on the hours of the week available <strong>to</strong> perform theWork and any related costs impacts based on a five (5) day schedule.I. Identify any discrepancies <strong>in</strong> the description of the work not <strong>in</strong>cluded <strong>in</strong> the RFP andyour approach <strong>to</strong> address those issues by go<strong>in</strong>g above and <strong>be</strong>yond the duty <strong>in</strong>perform<strong>in</strong>g the work. State any skills of the trade developed by your firm that setshigher standards <strong>in</strong> perform<strong>in</strong>g the work.J. Ensure at least one employee with each crew is fluent <strong>in</strong> English. The employees must<strong>be</strong> tra<strong>in</strong>ed <strong>to</strong> <strong>be</strong> respectful of personal property and set a high cus<strong>to</strong>mer servicestandard by adher<strong>in</strong>g <strong>to</strong> orders and <strong>in</strong>struction of the homeowner. Please descri<strong>be</strong> yourpolicies <strong>in</strong> work<strong>in</strong>g with homeowners or cus<strong>to</strong>mers.K. The selected Vendor will provide services related <strong>to</strong> the UC2B project and Consortiumwork<strong>in</strong>g on an “as needed” basis, work<strong>in</strong>g under the general supervision and report<strong>in</strong>g<strong>to</strong> the Interim Consortium Coord<strong>in</strong>a<strong>to</strong>r of the City of Champaign on <strong>be</strong>half of UC2B.Additionally, the selected Vendor will work closely with the City At<strong>to</strong>rney, the PolicyBoard, and other officials. Please provide a communication plan as <strong>to</strong> how you will keepall <strong>in</strong>formed and <strong>in</strong>volved <strong>in</strong> your work.L. Female and m<strong>in</strong>ority firms are strongly encouraged <strong>to</strong> submit <strong>proposals</strong>. The goodfaith efforts of all proposers, as evidenced by their Equal Opportunity <strong>in</strong> Purchas<strong>in</strong>gsubmittal and their proposal, will <strong>be</strong> closely reviewed <strong>to</strong> ensure that they have exercisedand cont<strong>in</strong>ue <strong>to</strong> exercise efforts <strong>to</strong> employ and utilize m<strong>in</strong>ority employees <strong>in</strong> theirbus<strong>in</strong>ess and on <strong>this</strong> undertak<strong>in</strong>g. <strong>All</strong> <strong>proposals</strong> <strong>shall</strong> <strong>in</strong>clude a statement of recentefforts <strong>to</strong> <strong>in</strong>clude m<strong>in</strong>ority and female employees <strong>in</strong> their bus<strong>in</strong>ess and efforts <strong>in</strong>promot<strong>in</strong>g <strong>in</strong>clusion <strong>in</strong> the profession generally.13Form A-1 10/09


XI.Cost ProposalThis section <strong>shall</strong> adequately descri<strong>be</strong> all of the costs for each section of repair andma<strong>in</strong>tenance functions from the time of contract execution through June 30, 2013.UC2B is look<strong>in</strong>g for competitive <strong>response</strong>s that understand the early stage nature and potentialimitations of the <strong>in</strong>itial project. The UC2B <strong>in</strong>itial deployment will <strong>in</strong>clude <strong>in</strong>stallation of 200anchor <strong>in</strong>stitutions and bus<strong>in</strong>esses and 2,500 residences. Additional sites may <strong>be</strong> added but arenot guaranteed as part of the <strong>in</strong>itial <strong>response</strong> and subject <strong>to</strong> change accord<strong>in</strong>g <strong>to</strong> the policiesand growth assumptions that will <strong>be</strong> determ<strong>in</strong>ed by the UC2B Policy Committee. UC2B islook<strong>in</strong>g for creative pric<strong>in</strong>g options that align with the execution and subscri<strong>be</strong>r take rate forservices. Creative approaches will <strong>be</strong> given additional consideration. Respondents <strong>to</strong> the RFPshould provide a description of their costs, bill<strong>in</strong>g and payment requirements for delivery ofservices.At a m<strong>in</strong>imum the proposal should <strong>in</strong>clude the follow<strong>in</strong>g pric<strong>in</strong>g detail and options available <strong>to</strong>UC2B:1) Cost for Repair and Trouble Resolution (Priority Basis)a. UC2B provided the Anchor Institution <strong>in</strong>stallers with an ONT, a Gateway, a fi<strong>be</strong>rservice box, a 3-meter fi<strong>be</strong>r patch cable, a fi<strong>be</strong>r patch cable <strong>to</strong> connect the OSPfi<strong>be</strong>r term<strong>in</strong>ation panel <strong>to</strong> the fi<strong>be</strong>r service box (if necessary) and two Category5e Ethernet cables for each cus<strong>to</strong>mer. The <strong>in</strong>staller supplied any other hardwarenecessary for the <strong>in</strong>stallation. The <strong>in</strong>staller ma<strong>in</strong>ta<strong>in</strong>ed a small <strong>in</strong>ven<strong>to</strong>ry ofmaterials supplied by UC2B such as fi<strong>be</strong>r optic and copper cables <strong>in</strong> variouslengths, as well as fi<strong>be</strong>r service boxes.D. The <strong>in</strong>staller <strong>in</strong>stalled the ONT and ran a series of UC2B–provided tests <strong>to</strong>confirm that it worked correctly with the core Active Ethernet equipment.E. The <strong>in</strong>staller configured the Gateway for both wired and wireless connections.Each cus<strong>to</strong>mer has one SSID for its private Wi-Fi use, and potentially a UC2BnetSSID for a second “community” Wi-Fi SSID. In some <strong>in</strong>stances it wasappropriate <strong>to</strong> core drill a basement or crawlspace wall <strong>be</strong>low grade. In other<strong>in</strong>stances it was appropriate <strong>to</strong> core drill a basement or crawlspace wall abovegrade. Structures built on a concrete slab most likely required drill<strong>in</strong>g a hole <strong>in</strong>the wall of the structure slightly above the slab or ground elevation depend<strong>in</strong>gon build<strong>in</strong>g style.B. Whenever the build<strong>in</strong>g penetration was above grade, a ¾” PVC conduit was used<strong>to</strong> protect the cable where it was exposed above grade and <strong>to</strong> turn the fi<strong>be</strong>rcable 90 degrees <strong>in</strong><strong>to</strong> the build<strong>in</strong>g. Under no circumstancesdid the locate wire orany metallic sheath<strong>in</strong>g enter the build<strong>in</strong>g. The Installer provided all exteriorrated PVC Conduit.17Form A-1 10/09


C. The removable outdoor protective sheath was removed from the UC2B provideddrop cable where it entered the build<strong>in</strong>g.D. The s<strong>in</strong>gle-strand <strong>in</strong>door-outdoor fi<strong>be</strong>r drop cable was term<strong>in</strong>ated <strong>in</strong><strong>to</strong> a smallfi<strong>be</strong>r service box, which was mounted <strong>to</strong> a baseboard <strong>in</strong> the build<strong>in</strong>g at a locationpredeterm<strong>in</strong>ed by the cus<strong>to</strong>mer dur<strong>in</strong>g the canvass by UC2B personnel. The fi<strong>be</strong>rservice box was supplied by UC2B.If the predeterm<strong>in</strong>ed location was not suitable, the Installer’s staff worked withthe cus<strong>to</strong>mer <strong>to</strong> determ<strong>in</strong>e a suitable location for the fi<strong>be</strong>r service box. <strong>All</strong> FTTPsites received UC2B electronics, and once the fi<strong>be</strong>r service box was <strong>in</strong>stalled, theInstaller used a UC2B-supplied 3-meter SC-<strong>to</strong>-SC s<strong>in</strong>gle strand fi<strong>be</strong>r patch cable<strong>to</strong> connect the Optical Network Term<strong>in</strong>al (ONT) <strong>to</strong> the fi<strong>be</strong>r service box.E.
