Read the following information carefully before filling in theAccreditation Application Forms Information must be either typed or clearly printed in block lettersIf you fill in the Application form manually, use a blue or a black ink pen.Do not write in the fields with the grey background, which are reserved for theCOJMP All Accreditation Application Forms have a univocal alphanumericidentification code Mistake corrections are accepted provided that they are clear and legible.The Application Forms cannot be kept or destroyed; they must be returnedwith the wording "Not Valid" on the front page. All empty fields in the application forms must be filled in using Romanalphabet block letters (English or French Alphabet), while Arabic numbersmust be entered in the number fields.(ABCDEFGHIJKLMNOPQRSTUVWXYZ)(0 1 2 3 4 5 6 7 8 9 ) A recent, high colour quality photograph (dimensions 35mm x 45mm) must beattached to any accreditation application.
