All Application Forms must contain the date, signature and stamp of the MediaOrganisation.The forms containing no stamp or signature shall not be accepted. Triplicate prints of the Accreditation Application Forms are required;two signed and stamped copies shall be faxed or e-mailed to the CNO; thethird copy must be kept by the applicant for registration The accreditation applications received after the deadline shall not be valid. Follow the detailed instructions contained in the attached User Guide to fillin the fields.B. DeadlineThe paper or electronic Accreditation Application Forms with the attachedphotograph must be received by COJMP by 24 th April 200914
3.3. Data entry in the Accreditation SystemThe COJMP will collect the Forms and enter the identification data in the system, toassign the rights, accesses and privileges according to the roles of the accredited persons,between February and April 2009.3.4. Form check and final approvalIn May 2009, after verifying the validity and correctness of the data recorded in theAccreditation data bank, the COJMP will make the necessary changes, if any, andelectronically send a "status report" containing the pre-accreditation data, thus startingan additional data correctness check before generating the pre-accreditation card.NOTE: The final report must be signed by the NOC of the home state forvalidation, stamped and sent back via express courier15
