3.1. Accreditation Application Forms are distributedAfter identifying the CNO’s contacts, the COJMP shall send the forms together with thefollowing, by February 28 th 2009:• Username and password to enter the Accreditation System on the Internet (seeattached letter)• Accreditation Manual for the appropriate (Media) group• User Guide for the Electronic Accreditation Program• Package “Consignment receipt” (ADC) to be filled in and immediately faxed back to0039/(0)85/4223676 or e-mailed to acr@pescara2009.it.The original document must be sent together with the filled-in Application Forms.3.2. Accreditation Application Forms are filled in and sent backThe Media contact points shall fill in the individual Accreditation Forms and fax or sendthem on line together with the Media Organisation authorization sheet, signed andstamped by the person in charge by 24 th April 2009 at the latest.The applications shall be sent to the CNO’s of the home state, which will then send all theMedia Accreditations to the Organising CommitteeHere follow some examples of Manual and Electronic Accreditation Application Forms9
