Coding accuracy35 The number of episodes with errors which would have changed theHRG was 30 representing 10 per cent of the total cases tested. The Trust’s HRG error rate hasvaried over the four year period of the programme. However, furtherimprovements in the accuracy of clinical coding could be made.39 The Coding Manager confirmed the electronic discharge summary waspoor in previous years but work has been done to improve this. The CodingManager held awareness sessions and regular meetings with clinical staff toexplain what details the coders need when coding. As the error rate hasimproved this indicated there have been problems in accuracy of the sourcedocument from using the discharge summary in isolation but this has nowimproved.
