OCCUPANTSOccupant(s) #Definition: The total number <strong>of</strong> injured and uninjured occupants in this motor vehicle involved inthe crash, including persons in or on the motor vehicle at the time <strong>of</strong> the crash.Indicate the number <strong>of</strong> occupants for each vehicle in the box.Rationale: Important for the <strong>of</strong>ficer at the scene to indicate how many people (injured anduninjured) are involved for reporting purposes. Used to evaluate the effectiveness <strong>of</strong>countermeasures that prevent or reduce injury and injury severity.DriverDefinition: The full name <strong>of</strong> the individual driver.Insert the full name <strong>of</strong> the person driving the motor vehicle at the time <strong>of</strong> the crash. If the persondriving the vehicle is licensed, the name should be exactly the same as shown on the driver license.If the driver’s true name is different from that shown on the license, the difference should beexplained on the back <strong>of</strong> the report in the description area. If the driver is unlicensed, indicate thefull name from other means <strong>of</strong> identification. Print the driver information in the following order:First Name Middle Initial Last NameIf the driver has no middle initial, then leave the area blank for the middle initial. DO NOT USEABREVIATIONS, SUCH AS NMN FOR ‘NO MIDDLE NAME.Rationale: This data element should be collected to corroborate the driver license number and t<strong>of</strong>acilitate linkage when names are available in the health and insurance files. When possible, obtainthis information from the driver license (via a bar code or “smart” license or via on-line linkage).Driver AddressDo not copy the address from the driver license unless it is acknowledged to be correct. Otherwise,obtain the street address or RFD number, apartment number, city, state, and zip code from someother source.Ask the driver to verify his or her address before entering it in the box. It is extremely important toobtain a correct phone number, Drivers License Financial Responsibility and the Courts may needto follow up with this number.Driver’s LicenseRationale: Necessary to evaluate the effectiveness <strong>of</strong> various licensing laws. This element is alsocritical in providing linkage between the crash and driver license files at the state level.24
