ExtricationDefinition: Extrication refers to use <strong>of</strong> equipment or other force to remove personsfrom the vehicles, i.e., more than just lifting or carrying person out <strong>of</strong> wreckage.Note: Please do not enter a code <strong>of</strong> 02 Extricated for helping a person from a vehicle. Extricatedwill generally require mechanical means such as a “jaws <strong>of</strong> life” or other extrication equipment.Enter the appropriate two-digit code for the driver and/or passenger(s) regarding extrication:01 Not Extricated02 Extricated99 UnknownNON-DRIVER PERSONS INVOLVED INFORMATIONNameDefinition: The full name <strong>of</strong> the occupant and/or pedestrian and/or bicyclist.Enter the names <strong>of</strong> the non-driver occupants and non-motorists involved in the crash in this space.Rationale: This data element should be collected to facilitate linkage when names are available inthe health and insurance files. When possible, obtain this information from the driver license.Date <strong>of</strong> Birth (DOB)Definition: The month, day, and year <strong>of</strong> birth <strong>of</strong> the person involved in a crash.Indicate the date <strong>of</strong> birth. This information should be taken from identification (if present),statement or other identification received in the investigation. The date <strong>of</strong> birth shall be listednumerically mm/dd/yyyy. Example: Month 06 Date 13 Year 1960If you do not have a date <strong>of</strong> birth put a dash in the box.Write the date <strong>of</strong> birth <strong>of</strong> the individual using the mm/dd/yyyy format.Rationale: Accurate reporting <strong>of</strong> date <strong>of</strong> birth is used to assess the effectiveness <strong>of</strong> occupantprotection systems for specific age groups, and to identify the need for safety programs directedtoward them. This element is also critical in providing linkage between the crash, EMS, andhospital records.67
