DO NOT USE ABREVIATIONS, SUCH AS NMNFOR NO MIDDLE NAME.If operator is under authority of a carrier, go to the Commercial Carrier Box, see page X, and usethe carrier name. Otherwise, obtain the information from the registration certificate, the driver, orother available sources.AddressVerify the address on the registration before entering information given by the driver. If theregistration information is incorrect, obtain the street address or RFD number, apartment number,city, state, and zip code and phone number from some other source.Always indicate: street, city, state, zip, and phone number including area code.
