USES OF THE CRASH REPORTSThe value <strong>of</strong> an investigation can only be measured by the ability <strong>of</strong> an <strong>of</strong>ficer to inquiresystematically into the crash to find out how and why the crash happened. An <strong>of</strong>ficer must have theability to report findings on the crash form so that the Law Enforcement Agency, <strong>Utah</strong> <strong>Department</strong><strong>of</strong> Transportation (UDOT), Drivers License Division (DLD), the <strong>Utah</strong> Traffic <strong>Safety</strong> Office,<strong>Department</strong> <strong>of</strong> <strong>Public</strong> <strong>Safety</strong>, the courts, and other agencies, who use the vital information gatheredat a crash, may obtain all the necessary information about the crash. Reporting must answerquestions about where and when the crash happened, who participated, what drivers and vehicleswere doing and intending to do, what kinds <strong>of</strong> vehicles were involved, and the how and why thecrash happened.The new <strong>DI</strong>-9 provides an efficient means to record the facts and circumstances <strong>of</strong> the crash.It is designed to consolidate the <strong>of</strong>ficer’s time and effort in obtaining and recording the facts <strong>of</strong> acrash.Each question is important to a variety <strong>of</strong> data users listed below.
