Restriction CodesDefinition: Restrictions assigned to an individual’s driver license by the license examiner.Enter the restriction code from the <strong>Utah</strong> Driver’s License. Restriction on other state licenses shouldbe coded as follows:A – No RestrictionsB – Corrective LensesC – Mechanical AidD – Prosthetic AidE – Automatic TransmissionF – Outside MirrorG – Daylight OnlyI – Limited OtherJ – OtherK – CDL Intrastate OnlyL – Vehicle Without Air BrakesO – 90 cc CycleQ – Limit EmploymentR – No Hazardous MaterialsU – 3 Wheel MotorcycleV – 40 MPH or LessW – MedicalY – LT 10K less than 10,000Note: If a driver has an out <strong>of</strong> state license, please adapt their code to fit the <strong>Utah</strong> codes listedabove.Rationale: Used to identify drivers with limitations on their operators’ license that have beeninvolved in crashes.Date <strong>of</strong> BirthDefinition: The month, day, and year <strong>of</strong> birth <strong>of</strong> the person involved in a crash.Indicate the date <strong>of</strong> birth. This information should be taken from the driver license or otheridentification received in the investigation. The date <strong>of</strong> birth shall be listed numericallymm/dd/yyyy. Example: Month 06 Date 13 Year 1960If you do not have a date <strong>of</strong> birth put a dash in the box.Rationale: Accurate reporting <strong>of</strong> date <strong>of</strong> birth is used to assess the effectiveness <strong>of</strong> occupantprotection systems for specific age groups, and to identify the need for safety programs directedtoward them. This element is also critical in providing linkage between the crash, EMS, andhospital records.AgeIndicate the age <strong>of</strong> the driver from the last birth date. Make sure the age <strong>of</strong> the driver corresponds tothe birth date on the driver’s license. Infants under one year <strong>of</strong> age are coded as 0. If you do notknow the age, please mark “U” for unknown.Law Enforcement ActionThe following boxes are used by law enforcement.Note: The citation number may be used to auto fill the <strong>DI</strong>-9 when using the FATPOT program.This will save time when filling out the form.26
