67EASTERN MEDITERRANEANPROGRAMME REVIEW GROUPOf the three endemic countries, Egypt and Yemen havestarted LF elimination activities. Egypt was one of thefirst countries to join GPELF in developing a nationalprogramme to eliminate LF as a public health problem,with the particular aim of reducing mf prevalence ratesto less than 1/1000. So far, the Egypt PELF has succeededin maintaining high MDA coverage rates. If such ratescan be sustained for the planned period, it is likely thatthe MDA programme will lead to the elimination of LFas a public health problem in Egypt. Preparatory activitieshave been carried out in the third endemic country,Sudan, but remain in abeyance for the time being. In2003 nearly 3 million people were covered by MDA. Thegeographical coverage of the Eastern MediterraneanPRG is shown in Figure 3.81.On the entomological side, a recent study in Egyptshowed that rates and intensities of mf ingestion andL3 production by mosquitoes fed on smear-negativetreated subjects were significantly lower than femalemosquitoes fed on smear-positive treated subjects. The distribution of thedisease is focal and it is the most important vector-bornedisease to cause a major public health problem in six governoratesof the Nile Delta, as well as in Giza and Assiutgovernorates in Upper Egypt. As a result of sustainedcontrol measures put in place by the Egyptian Ministryof Health and Population, however, most endemic villageshave low infection prevalence rates and intensity.Onchocerciasis is endemic in Yemen. The LF situation,however, was uncertain because few lymphoedema caseswere recognized, without confirmed laboratory diagnosis,so transmission was possible but uncertain. The leprosymission, a unit related to the Yemeni Ministry of PublicHealth, is also responsible for onchocerciasis control andprevention. As the mission has a well-developed networkof clinics throughout the country, it was chosen as theresponsible body for the national PELF.LF is endemic in Sudan, based on published and unpublisheddata from scattered spot surveys and hospital
