Many of the instructional provisions require certain actions by thevendor in offering a proposal. Addendum C Scope of Work, Experience, Delivery RequirementsProposer to present a simple proposal that demonstrates their experience and ability to perform therequired services and lists all tasks necessary to accomplish the required services including (but notlimited to) elements outlined in the Scope of Work and Delivery Requirements, and any requesteddeviations. Proposers are to provide sample utility bill and envelope with their proposal submission(reference paragraph 2.0.E.4); and provide a copy of their Data Security Policy with an explanationof how it will be applied to the Scope of Work provided under this contract (reference paragraph2.1.F.)Addendum D ReferencesProposer must provide three (3) references of current clients (within the past three years),specifying the name, address, telephone number, email address, type of services provided,and the name of contact person. Proposers may use the attached References form.Addendum E Non-Collusive Affidavit of ProposerProposer is to include the Non-Collusive Affidavit of Proposer with offer.Addendum F Business ComplianceThe Proposer must be in compliance with the laws regarding conducting business in the City ofWentzville and State of Missouri. The Proposer certifies by signing the signature page of thisoriginal document and any amendment signature page(s) that the Proposer and any proposedsubcontractors either are presently in compliance with such laws or shall be in compliance withsuch laws prior to any resulting contract award. The Proposer shall provide documentation ofcompliance within proposal submission. The compliance to conduct business in relation to this RFPshall include but may not be limited to:a) Registration of business name;b) Taxes (city/county/state/federal) upon request by Procurement Departmentc) Insurance (e.g. worker’s compensation/unemployment compensation) upon request byProcurement Department.Addendum G E-Verify Certification, Proof of U.S Citizenship Affidavit, and Affidavit of Participationin Federal Work Authorization ProgramProposer is to include the Proof of U.S. Citizenship Affidavit for identification purposes. Proposer isto include the E-Verify Memorandum of Understanding and the electronic signature page withintheir offer and the completion of the Proof of U.S. Citizenship Affidavit and the completion of theAffidavit of Participation in Federal Work Authorization Program.Page 12
