Hemophagocytic Syndrome จาก Epstein Barr Virus ทำให้เกิดระบบทาง ...

100ลัดดา สรคุณพิพิธกุล และคณะcerebellitis, Guilain-Barre, myeloradiculitis และ facial nervepalsy) 21 คน ตับอักเสบ 5 คน และอื่นๆ 15 คน อาทิเชน adultrespiratory distress syndrome (ARDS), thrombocytopenia,aplastic anemia, acute renal failure และ myocarditisพบวาผูปวย 39 คน ไดผลการรักษาดี โดย 27 คนไดผลดีมากและ 12 คนอาการดีขึ้นชัดเจน ในจำานวน 45 คนนี้ 35 คนไดรับการรักษาดวย acyclovir อยางเดียว 3 คนได acyclovir และfamciclovir รวมกับยาตานไวรัสตัวอื่น 3 คนได ganciclovirอยางเดียว 1 คนได ganciclovir รวมกับ foscarnet 1 คนไดfoscarnet และอีก 1 คนได vidarabine 25ผูปวยรายนี้มีขอที่นาสนใจคือ เมื่อไดรับการวินิจฉัยวาเปนEBV-associated HPC ก็มีพยาธิสภาพในปอดรุนแรงจนเกิดภาวะหายใจลมเหลว ซึ่งเกิดรวมกับภาวะตอมน้ำาเหลืองโตมากจนกดหลอดลม สำาหรับพยาธิสภาพในตอมน้ำาเหลืองพบวาเปน atypicallymphoid hyperplasia ซึ่งแยกจาก malignant lymphomaคอนขางยาก เมื่อไดรับ acyclovir ทางหลอดเลือดดำา รวมกับmethyl prednisolone และ IVIG ก็มีอาการดีขึ้นอยางรวดเร็วจนกลับมาเปนปกติคลายกับที่มีผูรายงานไวดังกลาวขางตน 24 ขอที่แปลกสำาหรับผูปวยของเราอีกประการหนึ่ง คือทั้งๆ ที่มี HPCแตผูปวยไมมีอาการแสดงทางตับ ซึ่งปกติจะตองเกิดกอนและรุนแรงกวาอวัยวะอื่น จึงเปนขอเตือนใจวา ในบางกรณีโดยเฉพาะ EBV ที่ทำาใหเกิดปอดอักเสบ และ HPC พยาธิสภาพทางตับอาจจะไมแสดงอาการชัดเจนได ทั้งๆ ที่เปนอวัยวะที่ EBV ชอบจูโจมเร็วและมากที่สุดกอนอวัยวะอื่นๆ การใช corticosteroid ในผูปวยของเรานอกจากจะเปนการรักษา HPC แลวยังชวยลดอาการทางชองปากและคอ โดยเฉพาะในกรณีที่มี การอุดกั้นทางเดินหายใจสวนบนได23References1.
