การใชยา vancomycin ในการรักษาโรคติดเชื้อ MRSAการใชยาบางขนานมีความจําเพาะกับเชื้อกอโรค ขนาดยาที่ใชตองเหมาะสมกับสภาพของโรคติดเชื้อและมีการตรวจหาคา MIC และระดับยาดวย เชน การใชยา vancomycin ในการรักษาโรคติดเชื้อ MRSA• ขนาดของยา vancomycin ในผูใหญคือ 15–20 มก./กก. /ครั้ง• ใหใชน้ําหนักตัวที่ชั่งได (actual body weight)• ใหยาทุก 8–12 ชั่วโมง• ใหไมเกินครั้งละ 2 กรัมในผูปวยที่มีหนาที่ไตปกติ• ในรายที่ปวยรุนแรง เชน sepsis, meningitis, pneumonia, infective endocarditis แนะนําให loading dose ขนาด 25–30 มก./กก. (actual body weight) ในการใหยาครั้งแรก• ถาขนาดยามากกวา 1 กรัมตอครั้ง อาจจะหยดยานาน 1-2 ชั่วโมง และฉีดยา antihistamineกอนจะให loading dose ไดLiu C, Bayer A, Cosgrove SE, Daum RS, Fridkin SK, Gorwitz RJ, et al.
