What is NEDSRA?
NEDSRA provides sports, recreation, and leisure
opportunities to adults and children with disabilities. By
working cooperatively with eight park districts and three
villages, NEDSRA serves more than 4,000 individuals each

Financial assistance is available for qualified applicants.

Registration Information
Important Dates:
August 29 – Early Bird Drawing
Register and pay by August 29 and you will
automatically be entered into a drawing to win a $50
NEDSRA program certificate! Each paid program
registration = one entry!
More programs = more chances to win!
Fall Registration Deadline:
September 6 (unless otherwise noted)
Register by deadlines to avoid cancelled programs.
Programs Begin: September 21

Find forms at the back of the brochure.
• Complete the Registration Form
Please remember to sign the bottom of the form.
• Complete the Annual Information Form
The Annual Information Form is valid for one year.
Once a form is on file, you do not need to fill one
out again until it has been in place for a year or if
there are changes in the status of information. The
Annual Information Form provides valuable details
to our staff, to assist in keeping each participant
safe and successful in our programs. Thank you for
your careful attention to this form and its details.
• Turn the Registration Form and Annual Information
Form, along with payment, to the NEDSRA Office
Once an Annual Information Form is on file, you
may also fax the form to our office or register over
the phone directly with our registrar. You may pay
the registration fees with a credit card at this
time, if you choose. If you would like to set up a
payment plan, please contact Nora Sandoval at
