What is NEDSRA?<br />

NEDSRA provides sports, recreation, and leisure<br />

opportunities to adults and children with disabilities. By<br />

working cooperatively with eight park districts and three<br />

villages, NEDSRA serves more than 4,000 individuals each<br />

year. Call to learn about our programs at 630-620-4500.<br />

Financial assistance is available for qualified applicants.<br />

Registration Information<br />

Important Dates:<br />

August 29 – Early Bird Drawing<br />

Register and pay by August 29 and you will<br />

automatically be entered into a drawing to win a $50<br />

NEDSRA program certificate! Each paid program<br />

registration = one entry!<br />

More programs = more chances to win!<br />

<strong>Fall</strong> Registration Deadline:<br />

September 6 (unless otherwise noted)<br />

Register by deadlines to avoid cancelled programs.<br />

Programs Begin: September 21<br />

Registration is Easy –<br />

Find forms at the back of the brochure.<br />

• Complete the Registration Form<br />

Please remember to sign the bottom of the form.<br />

• Complete the Annual Information Form<br />

The Annual Information Form is valid for one year.<br />

Once a form is on file, you do not need to fill one<br />

out again until it has been in place for a year or if<br />

there are changes in the status of information. The<br />

Annual Information Form provides valuable details<br />

to our staff, to assist in keeping each participant<br />

safe and successful in our programs. Thank you for<br />

your careful attention to this form and its details.<br />

• Turn the Registration Form and Annual Information<br />

Form, along with payment, to the NEDSRA Office<br />

Once an Annual Information Form is on file, you<br />

may also fax the form to our office or register over<br />

the phone directly with our registrar. You may pay<br />

the registration fees with a credit card at this<br />

time, if you choose. If you would like to set up a<br />

payment plan, please contact Nora Sandoval at<br />

630-576-4010.<br />
