STANDARD OPERATING PROCEDURES Standard Operating Procedure 3: Enrolment of Participants (CPA Standards: E3, E5) A copy of the Participants Manual is sent to all new participants with a participation form. This participation form must be completed and returned to the Scheme Secretary prior to the allocation of a unique code. All participation forms are scanned and saved on the EQA drive. STANDARD OPERATING PROCEDURES Standard Operating Procedure 4: Allocation of Code Numbers (CPA Standards: E4) Once a participant has agreed to the scheme they will be allocated a unique participation code, which is known only by the participant and the Scheme Secretary. The code allocated should be a three number code; for Consultant Pathologists it will be the next number in the sequence (for example if the last person to join the scheme was participant 192, then the next code would be 193). When adding trainees then the code should start 9—(e.g. 901, 902). When the new participant has been allocated a code they are entered onto the Name list, Code list and the current circulation list in the ‘Record of Responses.xls’ spreadsheet.
