engage new pharmacists in the organization, provide<br />

TSHP and to a new year of statewide<br />

collaboration. We want to<br />

thank everyone involved in the<br />

section for all your hard work and let<br />

you know it is greatly appreciated.<br />

We could not carry out our duties<br />

as a section if not for your efforts.<br />

We encourage all new<br />

practitioners throughout the<br />

organization and across the state to<br />

let your voice be heard through one<br />

avenue or another. We are all busy and have many<br />

things on our plate, but jumping in and contributing to<br />

your profession through a TSHP council or section is<br />

a rewarding experience you do not want to miss! It is<br />

truly amazing to see how everyone’s’ unique visions<br />

‘CAB’ Bond Memorial Research Grant<br />

Dr. Chester A. Bond (CAB) was a Professor of<br />

Pharmacy Practice and University Distinguished<br />

Professor for the Texas Tech University Health Sciences<br />

Center (TTUHSC), School of Pharmacy. <strong>The</strong> memorial<br />

grant named in his honor aims to assist pharmacist<br />

practitioners in hospitals and health systems, students,<br />

and faculty pursue original research and worthwhile<br />

projects that result in improving health outcomes for<br />

Texans. <strong>The</strong> 2017 recipient was “Utilization of Roboticassisted<br />

Video Conferencing to Extend <strong>Pharmacist</strong>s’<br />

Presence on Patient Care Floors to Provide Transitionof-Care<br />

Clinical Services in a Newly Opened<br />

Community Hospital” from Ran Xu, PhD, PharmD (PI)<br />

and co-investigators Kaye Rathmann, RPh, Kayleigh<br />

Emerson, PharmD, Karen Simpson, PharmD, Michael<br />

Liebl, PharmD (CO-PIs).<br />

In their proposal, investigators cite that within a<br />

few months since the hospital opening, pharmacists<br />

have discovered numerous medication errors during<br />

transition of care, including hospital admission and<br />

discharge, which can lead to adverse drug events<br />

(ADEs). In prior to admission (PTA) medication history,<br />

the errors can arise from various causes such as time<br />

constraints of nursing staff, patients or family being<br />

poor historians, and inaccurate medication records.<br />

This problem has become more significant with the<br />

advancement of the EMR, which requires the end-user<br />

to navigate a complex medication database. Thus,<br />

it is challenging for non-pharmacy personnel with<br />

limited medication management training to obtain and<br />

record accurate medication histories in EMR. Based<br />

on published literature and the principle investigators’<br />

previous research, pharmacist- or<br />

pharmacy technician-involved<br />

workflow on completing PTA<br />

medication history has been<br />

shown to reduce medication<br />

discrepancies, decrease<br />

potential ADEs, and improve<br />

provider compliance with<br />

medication reconciliation. <strong>The</strong><br />

team proposes a pilot study to<br />

utilize the robotic-assisted video<br />

conferencing (RVC) technology<br />

to provide pharmacists’ presence<br />

on patient care floors where<br />

clinical services are essential<br />

but lacking in a newly opened<br />

community hospital. <strong>The</strong> role of<br />

the virtual pharmacists includes medication history<br />

verification, discharge counseling, and addressing<br />

patients’ questions and concerns on medications. <strong>The</strong><br />

purpose of this project is to assess clinical and safety<br />

outcomes including the correction in PTA medication<br />

discrepancies and the improvement of patients’<br />

understanding of discharge medications after the RVC<br />

device has been deployed in our hospital.<br />

This 2017 Grant will be administered through<br />

Houston Methodist <strong>The</strong> Woodlands Hospital,<br />

Department of Pharmacy.<br />
