We want to thank everyone involved in the section for all your hard work and let you know it is greatly appreciated. We could not carry out our duties as a section if not for your efforts. We encourage all new practitioners throughout the organization and across the state to let your voice be heard through one avenue or another. We are all busy and have many things on our plate, but jumping in and contributing to your profession through a TSHP council or section is a rewarding experience you do not want to miss! Bond (CAB) was a Professor of Pharmacy Practice and University Distinguished Professor for the Texas Tech University Health Sciences Center (TTUHSC), School of Pharmacy. The memorial grant named in his honor aims to assist pharmacist practitioners in hospitals and health systems, students, and faculty pursue original research and worthwhile projects that result in improving health outcomes for Texans. The 2017 recipient was “Utilization of Roboticassisted Video Conferencing to Extend Pharmacists’ Presence on Patient Care Floors to Provide Transitionof-Care Clinical Services in a Newly Opened Community Hospital” from Ran Xu, PhD, PharmD (PI) and co-investigators Kaye Rathmann, RPh, Kayleigh Emerson, PharmD, Karen Simpson, PharmD, Michael Liebl, PharmD (CO-PIs). In their proposal, investigators cite that within a few months since the hospital opening, pharmacists have discovered numerous medication errors during transition of care, including hospital admission and discharge, which can lead to adverse drug events (ADEs). In prior to admission (PTA) medication history, the errors can arise from various causes such as time constraints of nursing staff, patients or family being poor historians, and inaccurate medication records. This problem has become more significant with the advancement of the EMR, which requires the end-user to navigate a complex medication database. Thus, it is challenging for non-pharmacy personnel with limited medication management training to obtain and record accurate medication histories in EMR. Based on published literature and the principle investigators’ previous research, pharmacist- or pharmacy technician-involved workflow on completing PTA medication history has been shown to reduce medication discrepancies, decrease potential ADEs, and improve provider compliance with medication reconciliation. The team proposes a pilot study to utilize the robotic-assisted video conferencing (RVC) technology to provide pharmacists’ presence on patient care floors where clinical services are essential but lacking in a newly opened community hospital. The role of the virtual pharmacists includes medication history verification, discharge counseling, and addressing patients’ questions and concerns on medications. The purpose of this project is to assess clinical and safety outcomes including the correction in PTA medication discrepancies and the improvement of patients’ understanding of discharge medications after the RVC device has been deployed in our hospital. This 2017 Grant will be administered through Houston Methodist The Woodlands Hospital, Department of Pharmacy. The 2018 application cycle will open in August. The Lone Star Pharmacist June 2018 Page 5 The Possibilities are Endless A message from the Chair of TSHP’s Membership Development Council TSHP Calendar of Events Being a Member of TSHP can be a very rewarding experience not only for professional advancement, but for one’s personal growth as well. As the Membership Council Chair, I have been able to connect with peers and colleagues in a way I could not have imagined. I have met so many people, made new friends, experienced various roles in separate committees, and have been a part of many new organizational initiatives. It has also provided me with tremendous insight into the profession of pharmacy practice at all levels. It is an extreme honor to be appointed for a second year as Chair of the Membership Council. The Jesse Chaffino great thing about it is, anyone of our members can do the same….and more! The Membership Council worked very hard this past year to grow TSHP by developing and offering new ways to bring value to our members. One new initiative was the creation of the Chapter Challenge. The goal was for our Local TSHP chapters to grow their membership while retaining current members, through new initiatives and local activities. It was a close race and congratulations go to the local El Paso Area Chapter for growing membership by a whopping 68% and to the Austin Area Chapter for achieving 99% retention! Obviously, people are joining TSHP and STAYING! Under the leadership of our next Executive Director, we look forward to continuing this trend and make it more exciting than ever to be a member of the TSHP community. The year culminated at our Annual Seminar in The Woodlands, TX, this past April. What a fantastic event and it only seems to get better every year! Pharmacists and technicians participated in the numerous events and CE courses. Students took advantage of the available scholarships and competed in the poster, clinical skills and disease state management competitions and the inaugural leadership challenge competition. Contributions to the R&E Foundation were at an all-time high and the PAC Auction was a great success. (I won Dallas Cowboys Tickets!) Networking was in full-effect, connecting people from all over the great state of Texas. Many of our community members were recognized for their achievements and contributions to the organization. We also had fantastic support from our Industry Sponsors and TSHP made sure to make it worth their while to attend. The core values of TSHP – Advocacy, Leadership, Fellowship, Mentoring, Education, Research – were on full display at the Seminar! The mission of TSHP is to connect, develop and advocate for the pharmacy profession to advance patient care. As a member-driven society, TSHP would not be able to do this without our community members. The organization can provide resources for you to get to where you want to be, both personally and professionally. If you’re not a member of the TSHP community, I invite you to join us! The experiences gained, skills learned, recognition received, and contributions made affect all we do as pharmacy and associate professionals. It’s a great way to enhance and fulfill our personal and professional goals, all while doing the right thing for our patients. Get involved! Grow with Us! Make new connections!
