Statement of Services
Key Questions: 1. What special education, related services and supplementary aids and
services are needed by the student to:
a. Attain annual goals?
b. Be involved and progress in the general education curriculum?
c. Participate in extracurricular and other non-academic activities?
d. Be educated and participate with other students with and without
disabilities?
2. What program modifications or supports are needed for school
personnel to enable the student to meet IEP goals, progress in the
general curriculum, participate in extracurricular and other nonacademic
activities, and be educated with disabled and non-disabled
peers?
3. Are the student’s special education, related services and supplementary
aids and services based on peer-reviewed research to the extent
practicable?

Definitions: • Special education means specially designed instruction, regardless of
where the instruction is provided, at no cost to the parents, to meet the
unique needs of a student with a disability. Special education may
include specially designed physical education, vocational education and
travel training.
• Related services means transportation and such developmental,
corrective and other supportive services as are required to assist a
student with a disability to benefit from special education.
• Supplementary aids and services means aids, services and other
supports provided in general education classes, other education-related
settings and in extracurricular and nonacademic settings, to enable a
student with a disability to be educated with nondisabled students to the
maximum extent appropriate.
