3.4.6 Vroege neuroborreliose: aandoeningen van het perifere zenuwstelsel
Aandoeningen van het perifere zenuwstelsel zijn beschreven bij patiënten met Lyme-borreliose in
de vorm van paresthesieën in de extremiteiten, polyneuropathie, mononeuropathie en radiculopathie
zonder meningitis. Bij deze patiënten werden zowel axonale 1,2 als ook demyeliniserende
afwijkingen 2,3 gevonden bij het elektromyografisch onderzoek. Zenuwbiopsieën toonden axonale
degeneratie, mononucleaire perivasculaire infiltraten echter zonder necrose van de vaatwand
maar wel trombose en rekanalisatie van capillairen van het endoneurium, perineurium en
epineurium. 4-6 De relevantie van deze bevindingen zijn onbekend. Deze afwijkingen werden
vastgesteld bij patiënten bij wie de diagnose Lyme-borreliose was gesteld op andere verschijnselen
dan de neuropathie.
Conclusie
Niveau 4 Er zijn geen gegevens over het voorkomen van neuroborreliose onder patiënten
met een poly- of mononeuropathie.

Bij patiënten met een radiculopathie die niet berust op een hernia nucleus pulposi of
herpes zoster, wordt expliciet gevraagd naar een recente tekenbeet, erythema migrans
(EM) of artritis. Bij patiënten met een positieve anamnese wordt aanbevolen Borreliaantistoffen
te bepalen in het bloed. Bij aanwezigheid van antistoffen in het bloed
wordt aangeraden onderzoek van de liquor cerebrospinalis te doen.

3.4.7 Chronische neuroborreliose
Chronische neuroborreliose is een zeer zeldzaam voorkomende aandoening en kan zich uiten
in de vorm van een langer bestaande encefalitis, encefalomyelitis, meningo-encefalitis of
radiculomyelitis.
Bij deze patiënten is altijd sprake van aanwezigheid van antistoffen in het bloed in combinatie
met intrathecale antistofproductie en veelal lymfocytaire pleiocytose. 1-4
Conclusie
Chronische neuroborreliose is een zeer zeldzaam voorkomende aandoening
die altijd gepaard gaat met intrathecaal geproduceerde Borrelia-antistoffen
Niveau 3 en veelal met een lymfocytaire pleiocytose.

Bij patiënten met verdenking op chronische neuroborreliose wordt aanbevolen om
antistoffen tegen B. burgdorferi in het bloed en de liquor cerebrospinalis te bepalen.
