Methoden: 7 Melodyklappen (6 in Pulmonalisposition, 1 in Trikuspidalisposition) wurden histologisch analysiert und die Ergebnisse mit 24 explantierten Pulmonalis-Conduits (Homograft, Contegra, Hancock) verglichen. Die Implantationszeiten der Melodyklappen betrugen zwischen 21 Monaten und 4,5 Jahren. Explantationsindikation war bei allen Prothesen in Pulmonalisposition eine Stenose (zweimal davon nach Endokarditis), bei derjenigen in Trikuspidalisposition eine Klappeninsuffizienz. Ergebnisse: Alle Klappentaschen der Melody-Prothesen waren zart. Die Entzündungsreaktionen waren bei den Melody-Explantaten nicht stärker ausgeprägt <strong>als</strong> bei den anderen Pulmonalis-Conduits. Grund für die überwiegend distal lokalisierten Stenosen war jeweils eine ungleichmäßige zellarme und bindegewebereiche Pseudointima-Proliferation, die nicht mit Stentfrakturen in diesem Bereich korrelierten. Verkalkungen waren bei Melody-Explantaten im Gegensatz zu anderen Conduits nicht nachweisbar. Thrombotische Auflagerungen fanden sich bei keinem der Melody-Explantate. Schlussfolgerung: Makroskopisch und histologisch fanden sich keine Auffälligkeiten der Klappentaschen. Die Pseudointima-Proliferation der stenotischen Melody-Prothesen erschien morphologisch übereinstimmend mit Proliferationen bei Homografts und anderen Conduits. Nach erfolgter Therapie wiesen auch die beiden Melody-Klappen nach stattgehabter Endokarditis keine vermehrten Entzündungsinfiltrate auf. VSD-Verschluss mit der Nit-Occlud Lê Spirale - erste Erfahrungen nach 18 Monaten Anwendung V53 A. Moysich, C. Hertwig, K.T. Laser, D. Kececioglu, N.A. Haas Herzund Diabeteszentrum NRW, Bad Oeynhausen, Klinik für angeborene Herzfehler/Kinderkardiologie Einleitung: Der interventionelle Verschluss von Ventrikelseptumdefekten (VSD) wurde bisher wegen seines erhöhten AV-Block-Risikos kontrovers diskutiert. In dieser restrospektiven Studie untersuchten wir die Effektivität und Komplikationen der Nit-Occlud-Lê-Spirale, die wir seit ca. 1 1/2 Jahren verwenden. Methoden: Wir berichten über 14 Kinder im Alter von 9 Monaten bis 12 Jahren (median 5 J.) mit einem Gewicht von 7,4 bis 48,7 kg (median 19,5 kg), bei denen nach angiografischer Darstellung und Messungen des VSD jeweils ein Device implantiert wurde, 8 in perimembranöse Defekte, 6 in muskuläre VSD. Der Verschluss wurde angiografisch dokumentiert, echokardiografische und EKG-Kontrollen fanden noch am Implantationstag, 48h später und nach ca. vier Wochen statt. Ergebnisse: Insgesamt wurden 12 Occluder (8x6mm, 10x6mm, 12x6mm) über eine 6F-Schleuse implantiert und 4 Occluder (14x8 mm) über eine 7F-Schleuse. Angiografisch wurde in sechs Fällen noch ein minimaler bis geringer Restshunt dokumentiert, der aber nach 48h echokardiografisch nicht mehr nachzuweisen war. In zwei Fällen trat passager eine Hämolyse auf, hier beendeten wir die postinterventionelle Heparingabe vorzeitig. In einem Fall trat direkt nach Implantation eines Devices in einen muskulären VSD kurzzeitig ein Rechtsschenkelblock auf. Diskussion: Im Gegensatz zu den anderen derzeit erhältlichen VSD- Occludersystemen besteht das Nit-Occlud-Lê-VSD-Device aus einer flexiblen Nitinolspirale, die sich gut an die Anatomie des jeweiligen Defekts anpasst. Hierdurch traten, auch nach vier Wochen, keine permanenten AV-Blockierungen auf. Die Lê-Spirale ist mit Polyesterfasern ausgestattet, um einen schnellen Defektverschluss zu gewährleisten. Dennoch besteht das Risiko der Hämolyse, die in unseren Fällen aber nur passager war. In humans, copy number variants of chromosome 1q21.1 have been associated with variable phenotypes comprising congenital heart disease including isolated TOF. In mice, the deletion of Gja5 can cause a variety of complex CHDs, in particular of the cardiac outflow tract, corresponding to TOF in many cases. In the present study, we screened for mutations in the GJA5 gene 178 unrelated probands with isolated TOF. A heterozygous nucleotide change (c.793C>T) in exon 2 of the gene leading to the p.Pro265Ser variant at the carboxyl-terminus (CT) of the protein was found in two unrelated sporadic patients, one with classic anatomy and one with pulmonary atresia. This GJA5 missense substitution was not observed in 1,568 ethnically-matched control chromosomes. Immunofluorescent staining and confocal microscopy revealed that cells expressing the mutant protein form sparse or no visible gap-junction plaques in the region of cell-cell contact. Moreover, analysis of the transfer of the gap junction permeant tracer lucifer yellow showed that cells expressing the mutant protein have a reduced rate of dye transfer compared to wild-type cells. Finally, use of a zebrafish model revealed that microinjection of the GJA5-p.Pro265Ser mutant disrupts overall morphology of the heart tube in the 37% (22/60) of embryos, compared to the 6% (4/66) of the GJA5 wild-type-injected embryos. These findings implicate GJA5 gene as a novel susceptibility gene for TOF. The study of the outflow tracts of the heart allows diagnoses of conotruncal malformation, that are not visible at the four-chamber view. AIM: The aim of this study is to assess the effects of the “extended-basic scan” of the fetal heart on antenatal diagnoses of CHDs in our Centre. METHODS: Between 2000 and 2010 2466 fetal echocardiographies were performed on 2090 pregnancies in our Centre. These were collected and divided into two groups (2000-2005 and 2006-2010). Both groups were analyzed and compared for diagnoses, indications, gestational age. RESULTS: The number of the diagnosed CHDs increased from 52 cases in the 2000-2005 period to 89 cases between 2006 and 2010. The number of malformations which proved to be detectable only through the study of the outflow tracts increased from 14/52 (27%) to 35/89 (39%). The indications for which patients were referred to fetal echocardiography changed in the period of time covered by the study. CONCLUSION: In our experience the introduction of the “extendedbasic scan” of the fetal heart improves the detection rate of CHDs as it increases the number of antenatal diagnoses of conotruncal malformations. Team-work is essential for physicians (obstetrician, cardiologist, pediatric, pediatric cardiac surgeon) in order to correctly diagnose CHDs and to correctly manage newborns with heart malformations.

Arterial duct (AD) stabilization by percutaneous stent implantation is nowadays widely considered as a valuable alternative to surgical shunt in patients with congenital heart disease and duct-dependent pulmonary circulation (CHD-DDPC). Aim. The aim of this study was to review clinical, procedural and follow-up data of a large single-centre series of neonates and infants submitted to AD stenting as palliation of CHD-DDPC. Methods. Between April 2003 and April 2012, AD stenting was attempted in 100 neonates with CHD-DDPC (n=14 pulmonary atresia intact ventricular septum, n=13 critical pulmonary stenosis, n= 73 complex congenital heart disease). Mean age of these patients was 25±50 days (range 3-255 days). Mean weight was 3.4 ± 1.1 Kg (range 1.2–8 kg). AD morphology was conical in 30 pts, tubular in 38 pts and tortuous in 32 pts. Results. Overall feasibility of the procedure was 93% (93/100). The procedure was not completed in 7 patient due to extreme ductal tortuosity. Mean procedural and fluoroscopy time were 128.8±59 min (range 60-240 min) and 26.5± 27 min, respectively.
