Case review<br />

14.21 The first case review must be held within 24 hours <strong>of</strong> <strong>the</strong> plan being opened. It<br />

must be chaired by <strong>the</strong> unit manager, who must appoint a case manager <strong>of</strong> minimum<br />

grade Senior Officer or Nurse Grade F. Each case must be treated individually. Care<br />

should be multi-disciplinary and reviews should involve people whom <strong>the</strong> person has<br />

contact with and/or who can contribute particular insight to <strong>the</strong> discussion or who <strong>the</strong><br />

prisoner feels comfortable with. Examples given are chaplains, members <strong>of</strong> <strong>the</strong> prison’s<br />

independent monitoring board, Samaritans, instructional <strong>of</strong>ficers and teachers (PSO<br />

2700, Annex 8G and ACCT form).<br />

14.22 The case review team must engage with <strong>the</strong> person as much as possible,<br />

consider all available sources <strong>of</strong> information, identify <strong>the</strong> person’s most urgent problems<br />

and <strong>the</strong> activities and people best able to provide support, and draw up a care and<br />

management plan (PSO 2700, Annex 8G).<br />

Care and Management Plan (CAREMAP)<br />

14.23 The care planning process emphasises <strong>the</strong> importance <strong>of</strong> engaging <strong>the</strong> person at<br />

risk and starting from that person’s perspective. The record <strong>of</strong> <strong>the</strong> plan prompts case<br />

managers to consider:<br />

Action to disable any suicide plan<br />

Action to link <strong>the</strong> person to people who can provide support<br />

Action to build on any strengths or interests <strong>the</strong> person may have<br />

Action to encourage alternatives to self-injury<br />

Action to reduce emotional pain caused by practical problems<br />

Action to reduce vulnerability because <strong>of</strong> mental health problems<br />

Action to reduce vulnerability because <strong>of</strong> drug/alcohol problems.<br />

14.24 Plans contain a record <strong>of</strong>: Issues (problems, resources, risk), Goals, Action<br />

required, By whom and when, and <strong>the</strong> Status <strong>of</strong> <strong>the</strong> action (for example, awaiting<br />

appointment).<br />
