Closure <strong>of</strong> <strong>the</strong> F2052SH<br />

3.11 The F2052SH form was closed at a review on 25 June 1998 attended by a senior<br />

<strong>of</strong>ficer and an <strong>of</strong>ficer. They had not attended <strong>the</strong> previous review. The summary <strong>of</strong> <strong>the</strong><br />

review says that Sonny said he had no domestic or prison problems, wanted to have a<br />

single cell, was willing to transfer to HMP Kirkham, and knew he could return to<br />

F2052SH status at any time just by asking. The section on support plan has been<br />

completed, apparently at <strong>the</strong> review meeting when <strong>the</strong> form was closed, and says that<br />

wing staff were to monitor Sonny’s behaviour, especially if he was in a single cell.<br />

Act <strong>of</strong> self-injury<br />

3.12 At 10.15 pm <strong>the</strong> next day, Friday 26 June, a nurse was called to Mr Lodge’s cell<br />

as he had slashed his arms deeply with a razor blade. According to <strong>the</strong> Inmate Medical<br />

Record, he would not let <strong>the</strong> nurse dress <strong>the</strong> wound and was verbally abusive. An<br />

ambulance was called and he was transferred to Warrington hospital.<br />
